- Please note that this is not a request for quotes or proposals; the Government is seeking information for market research purposes only. Your responses to the information requested will assist the Government in determining the appropriate acquisition method. A solicitation is not currently available. The Government may or may not issue solicitation documents. No formal solicitation document exists at this time. The Department of Veterans Affairs, Veterans Integrated Service Network (VISN 3), James J. Peters VA Medical Center, Network Acquisitions and Logistics program is conducting market research for qualified firms: Service Disabled Veteran Owned Small Business (SDVOSB); Veteran Owned Small Business (VOSB); 8A; HubZone and Small Businesses who are able to provide commercial ambulance services 24 hours, 7 days per week and have the ability and capability of meeting the Government's needs for companies who are in the business of ambulance services that include Basic Life Support (BLS) and Advanced Life Support (ALS). Vendor(s) need to respond to this request ONLY if you are capable and well experienced in providing this type of service to the NY Harbor Healthcare Facilities which includes the Bronx VA Medical Center located at 130 West Kingsbridge Rd., Bronx, NY 10468, Manhattan VA located at 423 East 23rd St., NY, NY 10010; Brooklyn VA located at 800 Poly Place, Brooklyn, NY 11209 & St. Albans Medical Facility located at 179 St. & Linden Blvd., St. Albans, NY 11425 and various Community-Based Outpatient Clinics (CBOC's). Vendor shall have all the certifications, licenses and permits required to provide the service that meet all federal, state, and local laws and regulations. Intended contract to be awarded on/about October 1, 2010. All responses shall include identification/certification of company size status and any pertinent information pertaining to this sources sought. Responses should include information about your company, prior ambulance experience and indicate if you have prior experience with VA hospitals, a service capability statement, DUNS number, point of contact and the size of your business pursuant to the following questions; (1) Is your business large or small? (2) If small, does your firm qualify as a small, emerging business? (3) If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(A) of the Small Business Act? (4) Is your firm a certified Hub Zone firm? (5) Is your firm a woman-owned or operated business? (6) Is your firm a certified Service-Disabled Veteran Owned or Veteran Owned Small Business? Provide a minimum of three "REFERENCES" for same or similar services. This is a market survey for informational and planning purposes only. THIS IS NOT A SOLICITATION AND DOES NOT OBLIGATE THE GOVERNMENT TO ISSUE A SOLICITATION. Please be advised that the Government will not pay for any information submitted under this announcement.
