- Medical Waste Collection and Disposal - Statement of Work This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation FA4877-11-Q-0055 is being issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through: Federal Acquisition Circular 2005-44 Defense Circular Number 20100802 Air Force Acquisition Circular 20100402 This procurement is being issued as a SMALL BUSINESS set-aside acquisition under NAICS code 562112 and the small business size standard is $12.5M. CLIN 1: Provide medical waste transportation and disposal services IAW attached statement of work. The requested services are being purchased for the 355 MDG on Davis-Monthan AFB. The requirement is being solicited as a firm/fixed price contract for one (1) base year with pricing for four (4) option periods. Service will be performed on Davis-Monthan AFB, Tucson AZ. Period of performance will be as follows: Base Year - 1 Oct 2010 to 30 Sep 2011 Opt 1 - 1 Oct 2011 to 30 Sep 2012 Opt 2 - 1 Oct 2012 to 30 Sep 2013 Opt 3 - 1 Oct 2013 to 30 Sep 2014 Opt 4 - 1 Oct 2014 to 30 Sep 2015 The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. 52.212-2, Evaluation -- Commercial Items applies. Quotes will be evaluated on price. Offerors must include a completed copy of the provision at 52.204-8 Annual Representations and Certifications and submit with its offer. All potential bidders are reminded, in accordance with FAR 52.204-7, Central Contractor Registration (CCR), that failure to register in CCR by Monday, August 23, 2010 by 12:00 pm PST will make an offeror ineligible for award. Contractor must also have NAICS code listed in this solicitation on their CCR Registration.
