- Performance Work Statement - Amended 6 August 2010 CDC Laundry Site Visit Meeting Minutes ***** This is an amendment to the Soliciation. Included in this amendment are the minutes from the Site Visit conducted on 3 August, 2011. Also included in this amendment is a revised Performance Work Statement. The following changes have been made: 1.) A description of the Pick-Up and Delivery Schedule has been added to 1.1.4. 2.) 1.1.4.1 - Pick up and Delivery for Regular Laundry including Minor Repairs has been take out of the Performance Work Statement. 3.) 1.1.4.2 - Pick Up and Delivery for Special/Expresss Laundrering has been changed to 1.1.4.1. 4.) On page 8, the Pick up and Delivery Dates have been changed. 5.) The Performance Work Statement is now 9 pages instead of 10 pages as before. Everything else remains the same in the Solicitation.****** This is the official issuance of Request for Proposal FA8601-10-R-0034 following issuance of the sources sought and presolicitation announcements. The Aeronautical Systems Center Operational Contracting Division is issuing the attached Request for Proposals (RFP) FA8601-10-R-0034 for Laundry Services for designated Child Development Centers (CDC) on Wright-Patterson Air Force Base (WPAFB), OH. The RFP consists of the attached documents as follows: Standard Form 1449 and continuation sheets, 28 Pages Attachment 1 to the SF 1449, FAR 52.212-1 - Instructions to Offerors - Commercial Items, 7 Pages Attachment 2 to the SF 1449, FAR 52.212-2 - Evaluation Factors - Commercial Items, 5 Pages Attachment 3 to the SF 1449, FAR 52.212-3 with Alternate I -- Offeror Representations and Certifications - Commercial Items, 17 Pages Attachment 4 to the SF 1449, DFARS 252.212-7000 - Offeror Representations and Certifications -- Commercial Items, 2 Pages Attachment 5 to the SF 1449, Performance Work Statement (PWS), 10 Pages Attachment 6 to the SF 1449, Department of Labor Wage Determination, 10 Pages The contractor shall provide all labor, facilities, transportation, services, equipment, and materials (except as identified in the PWS as Government furnished) necessary to provide laundry services for designated CDCs located on WPAFB, OH in accordance with the PWS. This is an acquisition of commercial services as defined in FAR 2.101 using simplified acquisition procedures as authorized by FAR Subpart 13.5, Test Program for Certain Commercial Items. Contract financing will not be provided for this acquisition. Award of one (1) firm-fixed-price contract with a 1-year base period with four 1-year options is contemplated. The proposed acquisition is a 100% Small Business Set-Aside. The NAICS Code is 812320 and the size standard is $4.5 million. This is a re-procurement of contract FA8601-05-P-0532, which expires 30 September 2010. The incumbent contractor is Long and Star Cleaners, Inc. of Miamisburg, OH, Commercial and Government Entity (CAGE) Code 45B32. The value of the current contract including all options, as of exercise of the fourth and final option period, is $183,666.25. A non-mandatory site visit is included in this solicitation and is to be conducted on Tuesday, 03 August 2010 at 01:30 P.M., Eastern Daylight Savings Time (EDST).
