- Health Program List Statement of Work This is a request for quotation for a Healthcare Improvement Consultant. Please see the attached statement of work for details about the requirements. The period of performance is 1 year from time of award. The provision 52.212-1, Instructions to Offerors—Commercial, applies to this acquisition. The provision 52.212-2, Evaluation—Commercial Items, applies to this acquisition. The following factors shall be used to evaluate offers: price, technical capability, and past performance. Technical and past performance, when combined, are significantly less important than price. The clause 52.212-4, Contract Terms and Conditions—Commercial Items, applies to this acquisition. The clause 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders—Commercial Items, applies to this acquisition. Quotes are due by August 16, 2010.
