Description
Attachment 3 - Experience Questionnaire Attachment 2 - Drawing of Project Attachment 1 - Davis Bacon Wage Decision Solicitation This project consists of excavation, bedding and backfill, and furnishing and installing replacement sewer line and clean out and patching with asphalt. Contractor is responsible for utility locates and safeguarding existing utilities. This project will consist of repairing a sewer line exiting building 2085 which repeatedly plugs. It is estimated that the sewer line is buried approxiamately 5 to 6 feet deep along the lateral line. The existing lateral line is expected to be 4" cast iron. The sewer lateral line plugs due to tree roots entering two 45 degree fittings which were improperly installed. Contractor is required to excate at the location of these two 45 degree fittings, remove and replace thse fittings in accordance with current code. Install a new clean out in the location shown on the attached sketch. Addionally, the contractor shall bed and backfill at these locations and patch the asphalt where it was removed. The dimensions shown in the attached sketch are approximate. At the contractors expense, the contractor may run a video camera inside the sewer line to determine the actual location of the 45 degree fittings. All work must be in compliance with the most recent international, state and local plumbing codes. The sewer line and fittings will be bedded in an appropriate sand bedding and backfilled and compacted. The moisture content of the backfill material will be adjusted to be suitable for compaction. Maximum lifts of backfill material shall be 6 inch. Each lift shall be compacted to at least 95% of maximum density. After the sewer line repairs are made, the asphalt shall be cut in neat straight lines and/or 90 degree angles prior to patching. No jagged patches will be accepted. Two inches (compacted) of hot mix asphalt will be placed over the excavated area. Compaction will be to a standard appropriate for areas used for highway vehicle traffic. Compact asphalt paving by rolling with a hand-operator roller, or equivalent, with a mass of at least one ton, or similar suitable method of compaction will be utilized. Perform initial compaction while the mixture is above 250 degrees F and continue compaction until no roller tracks remain. All removed pavement, large rocks, waste pipe, or other waste materials will be disposed off at an appropriate off site location. The contractor shall furnish all labor, transportation, equipment, materials, tools, supervision, and management as required.
