Description
RFQ for EOC Monitors/Wiring Solicitation Number: F2Z3C80189A001 Notice Type: Combined Synopsis/Solicitation Synopsis: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. F2Z3C80189A001 is being issued as a Request for Quotation, which incorporates provisions and clauses which are in effect through Federal Acquisition Circular 2005-43. This acquisition is 100% set aside for small business. The NAICS code is 443112 and the size standard is 500 employees. 1. DESCRIPTION OF SERVICES: The Contractor shall provide all necessary personnel, material, equipment, transportation and any other items and services necessary to install four (4) flat panel monitors, 3 55" LCD Samsung and 1 each 65" LCD Samsung and mounts, a matrix switcher, a wall-mount cabinet, audio receiver and a DVD player. Upon completion of installation and training the conference room will provide an increased viewing capability of the existing system as well as have the ability to permit interactive training. Installation will additionally provide the ability to project multiple video sources independently to each monitor. Current audio capabilities will not change. The Contractor shall comply with all federal, state, and local laws applicable to this commercial service. The Contractor shall keep in effect all necessary permits and licenses. 2. INSTALLATION: The Contractor will supply all necessary video cables, wall plates, connectors and hardware for connectivity. The installation will include wall mounting all brackets and monitors, wall cabinet installed and cable installation. The wall cabinet will contain the matrix switcher, audio receiver, DVD player, power supply, and utility drawer. Installation will be completed within three (3) business days of start date. Training will be scheduled upon installation completion and testing. Training is open to desired participants in one group session with a not-to-exceed two (2) hour window. **Installation and equipment should come with at least a standard 1 year commercial warranty** 3. REGULATIONS: The service must comply with the requirements of the National Electric Code and NFPA 101 Life Safety Code. 4. CONTRACTOR RESPONSIBILITY: The contractor will be responsible for providing any extended lengths of power, HDMI, LAN, connections for peripheral computer components, direct lines, cable connections, etc. as needed for the new configurations. 5. GOVERNMENT FURNISHED FACILITY, EQUIPMENT, PROPERTY AND SERVICE: The customer will provide electrical power at the installation site. The customer will also provide all 4 flat panel monitors, mounts, audio receiver, and DVD player, which will be used for this requirement. Base personnel will be responsible for disconnecting, relocating, and re-connecting computer equipment in the installation area during workstation assembly. 6. GENERAL INFORMATION. 6.1. HOURS OF PERFORMANCE: The contractor shall perform work during normal duty hours of 8:00 am to 5:00 pm, Monday through Friday. 6.2. CONTRACTOR EMPLOYEES: Contractor personnel shall present a neat appearance and be easily identified as contractor employees. The contractor shall obtain base identification and vehicle passes for all employees who perform work on McConnell AFB. Contractor personnel are required to wear or prominently display installation identification while visiting or performing work on the installation. The contractor shall not employ persons for work on this contract identified to the contractor by the Contracting Officer as a potential threat to the health, safety, security, general well being or operational mission of the installation and its population. Each employee will be easily identified by wearing distinctive clothing bearing the name of the company or by wearing appropriate badges that contain the company name and employee names. 6.3. SECURITY REQUIREMENTS: All personnel employed by the contractor in the performance of this contract, or any representative of the contractor entering the Government installation, shall abide by all security regulations of the installation. The contractor shall obtain the following pass and identification items for each employee as required (see AFFARS 5352.242-9000, Contractor Access to Air Force Installations). The contractor shall be provided all forms required for U.S. Government documents; the contractor shall complete and submit said forms in accordance with established DOD and Air Force directives, and shall ensure employees possess the documentation described paragraph 6.4. 6.3.1. Random Searches: Contractor personnel are subject to random personal and vehicle searches. If the contractor's personnel refuse to be searched, they will be denied entry to the base. Denial of entry to personnel who refuse to be searched, or detention of personnel found to be in possession of contraband, may result in loss of base driving privileges, debarment from the base, or other administrative action. 6.4. VEHICLES AND PASSES: Each vehicle must have a vehicle pass. The application for a vehicle pass must be accompanied by the state vehicle registration and proof of insurance. Vehicle passes must remain with the vehicle for which issued. Entrance to the base must be through the west gate. Contractor vehicles are subject to search prior to entering and exiting the base and while on the base. Further, the vehicles and equipment may be escorted during exercise operations and may be rendered immobile when not in operation by the removal of ignition keys or by other readily reversible means. 6.5 SAFETY INFORMATION: Contractors must be aware that they are performing work on a military installation and will fall under the direct control of armed Security Forces should the situation arise. 7. GOVERNMENT QUALITY ASSURANCE: The Government may use various quality assurance methods to determine the contractor's compliance with the SOW and the contract requirements. 8. GENERAL INFORMATION 8.1. HOURS OF PERFORMANCE: The contractor shall perform work during normal business hours of 7:30 am to 4:30 p.m. Local Monday through Friday, excluding all Federal holiday. 8.2. SECURITY REQUIREMENTS: All contractor employees shall carry contractor photo identification at all times. Contractor employees shall comply with any base security measures implemented at all times. This includes but is not limited to vehicle and goods searches and identification checks during times of increased security. The period of performance will be NLT 30 days after award of contract. The service will be performed at McConnell AFB. Award will be made to the Offeror who's proposal is determined to be the lowest price technically acceptable. A written notice of award or acceptance of an offer mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. AFFARS 5352.201-9101, Ombudsman (10 Aug 2005) (a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the party to another official who can resolve the concern. (b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMBC A-76 competition performance decisions).
