- SECTION J PART II SECTION E SECTION J PART I SECTION B SECTION J PART III SECTION G SECTION D SECTION J PART V SECTION M SECTION I SECTION F SECTION K SECTION L SECTION H SECTION J PART IV SECTION C - STATEMENT OF WORK This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Federal Bureau of Investigation (FBI) is issuing a Request for Proposal (RFP), on an unrestricted basis, open to both large and small business concerns, under solicitation number RFP-OSCU-124620A. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-44. The Government anticipates issuing a single contract award for each required location. Each contract will be a fixed price, indefinite-delivery indefinite-quantity type contract. The North American Industry Classification System code is 621111 with a small business size standard of $10.0 million. REQUIREMENT DESCRIPTION: The FBI requires the services of board-certified physicians capable of performing complete health examinations for personnel and occupational health services to FBI Field Office Locations in accordance with the attached statement of work. Services must be provided within a specific radius (refer to the Statement of Work) of the Division Headquarters or Resident Agency located in the following cities: Minneapolis, MN; Memphis, TN; and Springfield, IL. The Government will be required to order and the contractor required to furnish services for at least a minimum of $15,000.00 per contract over the life of each contract. In addition, if ordered, the contractor must furnish any additional services, not to exceed the maximum (non-guaranteed) limit of $1,200,000.00 per contract over the life of each contract. The anticipated period of performance of this contract will be for one (1) base year and four (4) possible additional one-year option periods. PROVISIONS: The following provisions apply to this acquisition: FAR 52.212-1, Instructions to Offerors - Commercial (JUN 2008) FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items (AUG 2009) FAR 52.212-4, Contract Terms and Conditions -- Commercial Items (JUN 2009), FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items (JUL 2010) Additional terms and conditions are contained within the attached solicitation documents, Sections B through M. Evaluation procedures are outlined in Part IV, Section M of the solicitation. SUBMISSION OF PROPOSALS: Proposals must be submitted in accordance with FAR 52.212-1, Instructions to Offerors - Commercial Items (June 2008) and the instructions listed in Section L of the solicitation.
