- FEDBIZZOPS Advertisement for West Linn, Oregon CBOC SOL# VA-260-10-RP-1041 The Department of Veterans Affairs is seeking approximately 5,800 to 6,707 square feet of clinic space in the Portland, Oregon Metropolitan South Area. The property shall be located within an area roughly square, described as follows: North Boundary - McVey Avenue Extended, East Boundary - Cascade Hwy Extended, South Boundary - Leland Road Extended and West Boundary - Stafford Road Extended. West Linn, OR CBOC Square Footage Requirement Description/Dimensions Approximate Square Feet Preferably the space shall be divided into the following estimated areas: a reception/waiting area, 12 exam rooms with nurse station, 10 offices, a 20' X 20' size room for Physical Therapy, 2 small telehealth rooms, 2 team meeting rooms, 2 new staff rest rooms and 2 patient rest rooms, a group room with central divider, 2 linen closets, a general storage closet and an SPD storage closet, an equipment closet space of 6' X 8' that contains a rack 8" away from the side of wall. It needs to be placed 4' from the back wall. Thermal dissipation needs to be able to handle 3200 BTU/HR. Room should be maintained at 65-70 degrees Fahrenheit. We need two 20Amp circuits dedicated for network equipment placed within 2' of rack on the side rack will be installed. Rack will need to be 19" rack with vertical wire management. Rack requires grounding. Restroom facilities shall be readily available within common areas of the building. The space must be compliant with the Americans with Disabilities Act. Parking, both handicapped and regular must be readily available. Convenient access to public transportation is required. All utilities including gas, electric, water, sewer and janitorial service shall be included in the lease. The facility must have a minimum of 30 dedicated parking spaces (in addition to parking spaces already provided). "Provide twelve (12) multipurpose examination rooms 120 NSF each. "Provide six (6) physician offices approximately 100 NSF each. "Provide four (4) Mental Health provider offices approximately 120 NSF each. "Provide one (1) nurse station approximately 150 NSF. "Provide one (1) Physical Therapy room approximately 400 NSF. "Provide two (2) Telehealth rooms approximately 60 NSF each. "Provide two (2) team rooms approximately 200 NSF each. "Provide one (1) group room, dividable in the center, approximately 400 NSF. "Provide two (2) patient toilets approximately 50 NSF each. "Provide two (2) staff toilets approximately 50 NSF each. "Provide one (1) patient waiting area approximately 400 NSF. "Each office and room will be lockable, have side door light switches, and have transom relights. "Exam rooms, offices and Telehealth room will be equipped with a minimum of two (2) wall outlets (quads), two (2) data outlets (quads) "Nurse station will be equipped with a minimum of four (4) wall outlets (quads) and two (2) data outlets (quads). "Locate the reception area immediately inside the main entrance of the space. "Provide one (1) SPD storage closet approximately 120 NSF. "Provide one (1) clean linen and one (1) dirty linen closet approximately 150 NSF each. "Provide one (1) small storage room approximately 75 NSF. Provide two (2) power outlets (quad) and one (1) data outlet (quad). "Provide one (1) LAN/WAN closet approximately 50 NSF (VA provided, VA installed equipment for network and VoIP system). Leaser to provide ventilation to closet. Closet must be lockable for security. "Provide adequate parking for employees and patients Exact square footage per delineated space/room to be determined. The duration of the proposed lease shall be a base period of five (5) years followed by five (5) one-year options.
