Description
The purpose of this contract is to provide for the availability of specialty services as follows, onsite cardiac consultation for Coatesville Veterans Affairs Medical Center (CVAMC) acute medical inpatients, 7 days a week. CVAMC inpatient consultations are to be completed on site within 24 hours of the initial request. Contractor will provide a board-certified or board eligible cardiologist(s) in the diagnosis and treatment of Cardiology conditions. The contractor shall provide specialized cardiac services including technical direction, oversight and general support of cardiology care. It is the responsibility of the specialist to ensure coverage is uninterrupted. Specialty services will be provided within the physician's granted privileges and will abide by the Medical Center Bylaws. CVAMC inpatient consults are to be completed on site within 24 hours of the initial request. When patients who present with chest pain are deemed to be at low risk (by virtue of their history, medical status, troponin testing, and EKG) are admitted to the CVAMC, the cardiology consultant will be contacted and is expected to examine and evaluate the patient by the end of the next day. The consulting cardiologist, on the completion of his/her onsite evaluation will advise the CVAMC attending physician regarding: (1)The likelihood and risk of an ACS syndrome; (2)Further treatment (including the need for the veteran to remain hospitalized); and (3)Further diagnostic evaluation. If the cardiology consultant recommends further diagnostic evaluation for ACS, the consultant will arrange for the testing to be completed within 48 hours. All emergency consultations will require an immediate response via telephone. At a minimum, documentation of the emergency consult will include diagnoses, treatment provided, prescriptions, any education provided, updates to the treatment plan, and recommendations for follow-up care. The consulting Cardiologist will be given access to CVAMC electronic medical records and will be expected to document a progress note in that system within 24 hours of the consultation. Patients may have open communication with the specialist and the CVAMC practitioner/provider regarding appropriate treatment alternatives. Likewise, open provider communication shall be maintained between the specialist and referring provider for the purposes of patient care coordination. The specialist will respond to all routine and emergency calls from CVAMC staff. The specialist will discuss cases requiring transfer from CVAMC to non-Veterans Affairs (VA) facilities with the VA treating provider/attending on duty. Any patient care incidents of a routine or non-routine nature will be reported to the CVAMC Chief of Staff. The specialist will be required to show evidence of malpractice and tort coverage within the limits specified by VA mandate. Should a "Stop Work" order be issued by CVAMC or the contract expires while a patient is in the process of receiving services. The Contractor will continue to provide services until that point in time when the patient is stable for transfer to VA facility able to provide continuation of services, or is stable for discharge. The determination of whether a patient is stable for transfer or discharge shall be left to the discretion of the treating physician. CVAMC shall pay for the patient ™s care and treatment according to this contract until patient is transferred or discharged.
