- SECURITY CHART TASK ORDER PRICING SHEET IDIQ CONTRACT PRICING SHEET PAST PERFORMANCE QUESTIONNAIRE FDA 3398 - PROTECT INFORMATION EPLC ARTIFACT DESCRIPTIONS FDA SMALL BUSINESS SUBCONTRACTING PLAN DESCRIPTION OF LABOR CATEGORIES TASK ORDER SOW IDIQ CONTRACT STATEMENT OF WORK (SOW) Terms and Conditions (Section 1: Contract Type (pg. 1); Section 2: Deliveries or Performance (pg.2); Section 3: Contract Administration Data (pgs. 3-6); Section 4: Requesting / Placing / and Processing Orders (pgs. 7-9); Section 5: Special Contract Requirements (pgs. 10-21); Section 6: Contract Clauses (pgs. 22-30); Section 7: List of Attachments (pg. 23); Section 8: Solicitation Provisions (pgs. 32-36): IDIQ Contract Instructions to Offerors (pgs. 37- 42), IDIQ Contract Evaluation Criteria (pgs. 43-46), Task Order Instruction to Offerors (pgs. 47-52), and Task Order Evaluation Criteria (pgs. 35-55). (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number is FDA-SOL-10-1071337 and is being issued as a "Request for Proposal" in accordance with FAR part 12 AND 15 procedures. (iii) This solicitation and incorporated provisions are those in effect through Federal Acquisition Circular 2005-44. (iv) The NAICS code is 541511 and the small business size standard is $25M. The Government intends to issue a an IDIQ contract with a 60 month ordering period and one firm fixed price task order with a 12-month base period and four 12-month Option Periods. FAR Provision 52,217-5, Evaluation of Options applies to this acquisition. Each Request for Task Order Proposal (RFTOP) issued under this contract will identify the Government's determination of task order type. All questions must be submitted in writing to Jody.O'Kash@fda.hhs.gov no later than Wednesday, August 11, 2010 at 2:00pm. Answers to all questions will be posted as an amendment to the solicitation. No calls please. Volumes I, II, and III of the contractor's proposal shall be submitted in accordance with the instructions in the attached solicitation, no later than August 18, 2010 at 2:00pm.
