Description
SCOPE OF WORK 1.Contractor will furnish all labor, equipment and secure HIPPA compliant consoles to provide control and destruction of classified and patient privacy information on site at the VA Medical Center Community Based Outpatient Clinic, 323 Upper River Road, Gallipolis, Ohio 45631. 2.Contractor is responsible for maintaining liability insurance for the duration of the contract and for following all pertinent federal and state regulations with regard to transportation and destruction of classified medical documentation. 3.The contractor shall supply two (2) HIPPA compliant (lockable) consoles with slotted lids allowing for the discarding of patient sensitive documents. The security containers must be aesthetically compatible with the facility's interior design scheme. 4.In addition, contract will also provide for destruction of plastic patient sensitive information items generated on site. This may include patient armbands, pharmacy prescription bottles, IV bags, etc. 5.The contractor is required to remove and discard documents from each container every other month from the facility listed. Destruction services will be conducted during regular business hours: 8:00 a.m. - 4:30 p.m., Tuesday or Thursday. 6.The contractor will destroy all paper products removed from the outpatient clinic in accordance with VA Directive 3671, Destruction of Temporary Paper Records, and/or as approved by local VA authority, thereby rendering the paper completely unreadable. All destruction will be performed on site. 7.Per VA guidelines, all paper material within the scope of this contract must be submitted for recycling and the Contractor will provide suitable documentation substantiating this fact. 8.The VA shall not be required to separate documents by color or grade of paper, nor remove paper clips or other fasteners. 9.The contractor will provide a Certificate of Destruction at the completion of each service visit to the designated VA contact person. 10.The contractor personnel shall be in uniform and display photo identification at all times. 11.The contractor will coordinate container location and shredding schedule with the COTR. The contract personnel must check-in with designated person prior to removal of any documents. 12.The Gallipolis CBOC Point of Contact will assist in providing a designated parking location for the contractor. 13.In accordance with: a. American Health Information Management Association, (AHIMA). b. Centers for Medicare and Medicaid Services (CMS). c. VHA Directive 2003-028 "Compliance and Business Integrity Program", its updates or replacement directives, d. HHS OIG Compliance Program Guidance for Hospitals, as published 63 FR 8987 (February 23, 1998). e. HHS OIG Supplemental Compliance Program Guidance for Hospitals as published at 70 FR 4858 ( January 31, 2005) - the contractor is required to provide to the Medical Center a copy of their internal Compliance Program, Training and Monitoring Plans. This is also to include documentation of training and monitoring procedures.
