This acquisition has not been set aside for any small business category of socio economic concern. Description of Services: Clin 0001: Physician, Nursing and Ancillary services to support the Rodriguez Army Health Clinic, Fort Buchanan, PR, Period of Performance: 1 October 2010 30 September 2011, Clin 0001AA, Nursing Assistant, 1 FTE, 2000 hours; Clin 0001AB, Licensed Practical Nurse, 1 FTE, 2000 hours ; Clin 0001AC, Psychiatrist, 1 FTE, 2000 hours; Clin 0001AD, Nurse Case Manager, 1 FTE, 2000 hours; Clin 0001AE, Medical Clerk Claims, 1 FTE, 2000 hours; Clin 0001AF, Psychologist, 1000 hours; Clin 0001AG, Family Physician, 2 FTEs, 4000 hours; Clin 0001AH, Medical Clerk, 1 FTE, 2000; Clin 0001AJ, Medical Liaison, 1 FTE, 2000 hours; Clin 001AK, Social Worker, 1 FTE, 2000 Hours; Clin 0001AL, Army Substance Abuse Coordinator, 1 FTE, 2000 hours; Clin 0001M, X-Ray Technician, 1 FTE, 2000 hours; Clin 0001AN, Medical Technologist, 1 FTE, 2000 hours. Four options will be included for the same services and quantities that may extend contract performance through 30 September 2015. The formal request for proposals shall be issued shortly after this notice is posted and it is anticipated that offers will be accepted through 16 August 2010.
