- Washer and Dryer Soliciation Wage Determination Performance Work Statement Past Performance Questionnaire This is a solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a written solicitation will not be issued. Submit only written quotations for Solicitation FA4800-10-T-0016. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-26. The following factors shall be used to evaluate offers: 1. Past Performance and 2. Price. Past Performance is significantly more important than Price when evaluated. This is a 100% Small Business Set-Aside. The associated NAICS code is 532210 with a $7.0 size standard. This requirement is to lease commercial grade washers and dryers (electric) in accordance with the attached Performance Work Statement. FOB point is Destination. All proposals must be faxed or emailed to POC Ursela C. Jackson at 633 CONS/LGCZ, 74 Nealy Avenue, Langley AFB, VA 23665-2088. Quotes must be signed, dated and submitted by 10 August 2010, 1200 pm, Eastern Standard Time (EST) to the 633d Contracting Squadron/LGCZ, Attn: Ursela C. Jackson, Contract Administrator, Commercial Phone 757-764-4923, Fax 757-764-7447, E-mail ursela.jackson@langley.af.mil. All contractors must be registered in the Central Contractor Registration http://www.ccr.gov database prior to any contract award. Please submit the following information with each quote: Cage Code, DUNS number, Tax Identification Number, Prompt Payment Terms, Delivery Time, GSA Contract No. (if applicable), Date offer expires, warranty, Line item unit price, and Total cost.
