- The USPFO for Utah intends to issue a sole source award to Benson Medical Instruments Company, 310 4th Avenue South Suite 5000, Minneapolis, MN 55415-1025 to provide the following supplies/equipment: (1) Twelve each, P/N 500303-005 CCA-200M Expansion Audiometer; (2) Twelve each, P/N 500132-001 BAS-200 Simulator; (3) FOB Destination to Utah National Guard Camp Williams, AGCW Bldg. 9000, 17800 South Camp Williams Road, Riverton, UT 84065. The Benson Medical BAS-200 and CCA-200M series audiology equipment provides standardized audiology exams, which all feed into an existing Army-wide Force Health Protection database. Benson Medical Instruments equipment is standardized for all Dept. of Army Audiology Programs. Any firm that believes it can provide the stated supplies/equipment is invited to submit a written notification to USPFO for Utah within three (3) calendar days from the date of publication of this notice. Supporting information must be furnished to demonstrate the firms ability to provide the stated supply/equipment. Information can be sent to the Contract Specialist, Ms. Jennifer Shaw at jennifer.shaw5@us.army.mil. No telephone or facsimile requests will be accepted. Responses received will be evaluated; however, a determination by the Government not to compete the proposed procurement based upon responses to this notice is solely within the discretion of the Government. If no responses are received, USPFO for Utah will proceed with the sole source award.
