Description
The Department of Health and Human Services/ Division of Acquisition Management intends to negotiate on a sole source basis (FAR 6.302) with CHD Research Associates, Inc. of 2300 Clarendon Blvd. #1400, Arlington, VA 22201 as the only responsible source that can provide the service to conduct an in-depth analyses of factors affecting all-cause and potentially avoidable hospitalizations among health patients using analytic files derived from CMS source data files. The services required are as follows: • Earlier work consisted of analytic files derived from CMS source data files and, using these files, of national estimates of hospitalization and potentially preventable hospitalization (using Prevention Quality Indicator (PQIs) methodology) for 2001-2007 among home health patients. These estimates were carried out for basic demographic subgroups of patients (e.g., age, race, Medicare status, and prior hospitalization). The contractor will explore in-depth additional factors affecting hospitalizations (e.g., functional and cognitive impairment, living situation, comorbidities, surgical and medical diagnoses in prior hospitalizations) exploring patient level and agency- level factors (e.g., start of care, frequency of visits in 1st two weeks of episode, type of care received). • Using the latest available calendar years of home health patient level files containing the Outcomes and Assessment Information Set (OASIS) assessment data merged with Medicare claims data, determine the association between all-cause and PQI –related hospitalization rates for additional home health subgroup users – for example, patients with different levels of functional and cognitive impairment, and users living in different geographic regions or being served by different types of agencies/facilities, agency process measures, and for racial/ethnic groups used in the NHQR/NHDR (sample size permitting). • Develop refined predictive models for all-cause and potentially preventable hospitalizations (e.g., logistic and/or other multivariate regression), as appropriate, controlling for patient characteristics, regional market factors (e.g., including Dartmouth index for available years, HCC) and other variables, including agency process measures (types of home visits, front-loading (number of visits in first 2 weeks), start of care). This acquisition is being conducted under simplified acquisition procedures. This is a Small Business set-aside. The intended procurement will be classified under North American Industry Classification System (NAICS) 541611 with a Small Business Size Standard of $7.0 Million. This notice of intent is not a request for competitive proposals and no solicitation document exists for this requirement. However, parties interested in responding to this notice shall submit technical data, including cost, sufficient to determine capability in providing the same services. All capability statements received by closing of this notification of this synopsis will be considered by the Government. Each response should include the following Business Information: a. DUNS. b.
