- Synopsis: Added: Mar 15, 2010 11:28 am The 374 Contracting Squadron seeks to identify potential sources that are capable of providing debt collection services. REQUIREMENTS: The 374th Medical Group, Yokota AB, Japan is looking for help with collecting debts from non-military members and their dependents that have received medical service at Yokota AB hospital but have left the premises without making payment. ACTION ITEM: The 374th Contracting Squadron is looking for contractors with the above capability along with their approach to terms, proposals, contracting vehicles and any other factor within this arena. Interested sources that believe they have the capability to perform the services required, should submit a capability statement to 1LT Marco Campos, Contracting Specialist, via e-mail at marco.campos@yokota.af.mil or Merry K. Sargent-Green, Contracting Officer, via e-mail at merry.green@yokota.af.mil in. Responses shall be marked "Medical Collection", CAPABILITY STATEMENT, and shall be received no later than 12:00PM EST on July 21, 2010. The North American Industry Classification System (NAICS) code for this requirement is <U>561440</U>. The size standard for this NAICS code is $7 million. Responses should be limited (not to exceed 3 pages) but clearly state the capabilities of the company to perform tasks above. The capability statement should also include Central Contractor Registration (CCR) information (website: http://www.ccr.gov). The size limitation for e-mail attachments is 5 megabytes. MS Word 2002 is the standard word processing software. NOTE: All questions must be submitted in writing to 1LT Marco Campos or Merry K. Sargent-Green via e-mail address listed above.
