[] (2) FAR 52.222-42, Statement of Equivalent Rates for Federal Hires (May 1989) (29 U.S.C. 206 and 41 U.S.C. 351, et seq.). 3) Page 47 SECTION D - CONTRACT DOCUMENTS, EXHIBITS, OR ATTACHMENTS Delete all reference to Department of Labor-Wage Determination as they do not apply to Bulk Oxygen as a supply under NAICS 325120 4) Page 52 FAR 52.212-2 EVALUATION--COMMERCIAL ITEMS, Factor 3 Add following Note: Offeror will be required to submit Statement of Affirmation upon notification of award. When submitting your technical proposals add the statement œStatement of Affirmation will be submitted upon award notification by medical center. œ This will also be included in instruction provided to the Technical Evaluation Board (TEB).
