This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This procurement is being conducted under Simplified Acquisition procedures pursuant to the authority of FAR Subpart 13.5, Test Programs for Certain Commercial Items (10 U.S.C. 2304(g) & 2305 & 41 U.S.C. 253(g) & 253a & 253b), FAR Part 12, Acquisition of Commercial Items (Title VIII of the Federal Acquisition Streamlining Act of 1994 (Public Law 103-355)), & FAR Subpart 37.4, Non-personal Health Care Services (10 U.S.C. 2304 and 41 U.S.C. 253). This announcement constitutes the only solicitation; therefore, a written solicitation will not be issued. The Aberdeen Area Indian Health Service (IHS) intends to award a firm-fixed price requirements type contract in accordance with the Request for Proposal (RFP) 11-04, for an onsite Radiologist to perform services at the Pine Ridge PHS, IHS Hospital in Pine Ridge, South Dakota. The solicitation is issued as a 100% Service Disabled Veteran Owned Small Business set-aside. The solicitation documents & incorporated provisions & clauses are those in effect through Federal Acquisition Circular 2005-44. The associated North American Industry Classification System code is 621111and the small business size standard is $10 million. PRICE SCHEDULE: RADIOLOGIST SERVICES: the quoted pricing must be all inclusive (to include but not be limited to travel, lodging, per diem, fringe benefits, federal, state and local taxes) plus all other costs pertinent to the performance of this contract. No differential pay or holiday will be granted; this is an all-inclusive price. The contract resulting from this solicitation will not obligate funds; funds must be obligated prior to starting performance through a delivery order or task order. Orders may be submitted electronically, facsimile or hard copy. Please complete the following estimated pricing schedule utilizing your most competitive and reasonable rates which the IHS will use to the "Best Value" to the government. 1. Please provide an hourly all inclusive flat rate BASE YEAR (10/1/2010 through 7/31/ 2011) Approximately 1670 hrs @ $ per hour_________________ ; 2. OPTION YEAR ONE (8/1/2011 through 7/31/2012) Approximately 2040 hrs @ $ per hour_________________; 3. OPTION YEAR TWO (8/1/2012 through 7/31/2013) Approximately 2040 hrs @ $ per hour_________________; 4. OPTION YEAR THREE (8/1/2013 through 7/31/2014) Approximately 2040 hrs @ $ per hour_________________; 5. OPTION YEAR FOUR (8/1/2014 through 7/31/2015) Approximately 2040 hrs @ $ per hour_________________; PURPOSE OF THE CONTRACT: C-1 Purpose of the Contract The mission of the Pine Ridge Indian Health Service Hospital is to provide the best possible health care services to the Native American population on the Pine Ridge Oglala Sioux Reservation in South Dakota. To meet the mission, the service unit requires contractor support to assist the Service Unit in the placement of a licensed Radiologist into a temporary duty assignment in the Pine Ridge IHS Hospital. The care provided by the Contract Radiologist shall be as comprehensive as the Indian Health Service supplied facilities, equipment, and support services permit. The Indian Health Service requires that Radiology services continue without interruption during the term of this contract. The contract resulting from this solicitation shall: 1. Be a Firm-fixed price, Non-Personal Healthcare Service Requirements Type Contract for Radiologist Services. 2. Allow the Government to evaluate the quality of professional and administrative services provided. 3. Require the Contractor to indemnify the Government for any liability producing act or omission by the Contractor, its employees and agents occurring during contract performance; 4. Period of Performance: *Base year 10/1/2010-7/31/2011 @ approximately 1750 hours of services to be provided. *Option year one (1) 8/1/2011-7/31/2012 @ 2040 hours of services to be provided. *Option year two (2) 8/1/2012-7/31/2013 @ 2040 hours of services to be provided. *Option year three (3) 8/1/2013-07/31/2014 @ 2040 hours of services to be provided. *Option year four (4) 8/1/2014-07/31/2015 @ 2040 hours of services to be provided. Contractor's agreement is hereby incorporated by reference as part of this Requirements Contract. In the event of conflict between any terms of the agreement and the Requirements Contract, applicable federal laws and statues will prevail. This Requirements Contract does not obligate any funds. Funds will be obligated as individual task orders are placed hereunder. Prior to the time that services are needed an individual task order will be placed against this Requirements type contract and shall be submitted electronically, facisimile or hard copy. Individuals authorized to place orders against this Contract are: 1) Lara Nelson 2Terry Campos 3) Michelle Tibbits The Individual providing services under this Requirements Contract, which involve regular contact with or control over Indian children, are subject to a character investigation in accordance with the Indian Child Protection and Family Violence Prevention Act P.L. 101-630. The contractor must perform in accordance with section 231 of the Crime Control Act of 1990, P.L. 101-647. Failure to comply with these statutes will result in immediate removal. Within the first two (2) weeks from the effective date of award a Childcare National Agency Check and Inquiries (CNACI) suitability screening will need to be performed and cleared. The following form will be mailed to the awardees for completion. Please return this form to the service unit within the first two weeks from effective date of award. (a) One FD-258 Fingerprint chart The selected offer must complete the Standard Form 85 Questionnaire for Non-Sensitive Positions electronically. Awardees must make arrangements with the service unit background investigation coordinator, Annabelle Blackbear or Alsie LeBeau within the first two weeks from effective date of award. C-2. Government Furnished Property, Facilities, and Services The Service Unit shall provide office space including a desk and chair for reviewing and interpreting patient images on the Centricity PACs system. Reports will be dictated by the Radiologist with Voice Recognition dictation or a dictating system that is available. In the provision of medical services, the Contract Radiologist will be authorized to use all areas of the medical treatment facility available to civil service and commissioned personnel in similar positions. The same restrictions apply to the contract Radiologist as any other Indian Health Service personnel to use these areas for official business activities only. The Contract Radiologist will be authorized to use all administrative and ancillary support available to Indian Health Service personnel. This will include but not be limited to copy machines, fax machines, medical library, Internet, and telephone service. The same restrictions to use these items for official Indian Health Service business apply. The Service Unit shall provide all required blank Government forms applicable for documentation in the Indian Health Service patient record system. Applicable forms shall include Patient Care Component (PCC) forms and Service Unit charge tickets. An appropriate orientation to the necessity of such forms will be provided by Service Unit personnel. The Pine Ridge Service Unit shall assume the responsibility of patient care coordination. The service unit shall provide a multidisciplinary approach in order for patients to receive the best care possible. Making all the necessary appointments for patients, encouraging them to keep their appointments, and following up and implementing recommendations are all necessary parts of the care coordination activity. The Service Unit shall encourage patients to be independent, well informed, and active participants in their own healthcare. Therefore, coordinating care must balance between fostering this independence and ensuring that patients receive optimal and appropriate healthcare. C-3. Contractor Furnished Equipment, Facilities, and Services The Indian Health Service does not anticipate a need for contractor furnished equipment. C-4. Statement of Work Radiologist Duties and Responsibilities at Pine Ridge include, but are not limited too: 1. The Contractor will assure availability of professional Radiologist Services, Monday through Friday, 8:00 am to 4:30 pm to provide film interpretations and reports, including exams done at night and over the weekends, consultations and/or in-services to the Medical providers on staff, as well as performing fluoroscopic and special procedures as needed by the Pine Ridge Radiology Department. The Radiologist shall provide a high standard of expertise contributing to the provision of efficient and effective medical services to the Indian community served by the Pine Ridge IHS facility. 2. The Radiologist will have direct involvement with performing and interpreting Ultrasonography, including carotids and venous dopplers, Cat Scan to include soft tissue necks and bone desities and arthrograms, i.e. shoulders. Scoliosis Series will also need to be read. 3. The Radiologist will be responsible for interpreting all mammogram studies provided on site, therefore, must be MQSA certified. If a digital Mammography unit is available, the radiologist must be certified to read digital mammography. If a non MQSA certified Radiologist is provided, it must be approved by the appropriate personnel at the Hospital, prior to sending them. 4. Radiologist will monitor Pyelography, administer contrast media and assure that the IV Pyelography technique is adequate for evaluation of the urinary tract. 5. Radiologist will notify the Radiology Supervisor concerning any technical or administrative problems that may occur. 6. The Radiologist will interpret and assure that final interpretation results are accessible from the IHS Radiology package within 48 hours of the time they are received. All critical finding will be reported to the appropriate care provider by the Radiologist in a timely manner. 7. The Radiologist will assist Pine Ridge Hospital in every way possible to achieve and maintain a high level of patient service with efficiency and economy consistent with the highest standards of quality patient care. 8. The Radiologist will assist Pine Ridge IHS in attaining and maintaining JCAHO, HCFA, ACR and FDA standards by: a) Evaluating and providing advice regarding the Performance Improvement studies done regularly by the Pine Ridge Radiology Department. b) Provide direct input to the Medical staff to enhance the Radiology services at Pine Ridge. Additional duties will include: a) Notifying the head of Radiology and the Clinical Director of significant technical or administrative problems interfering with or threatening patient care in an way. b) Monitoring the technical aspects of every radiograph given to the contractor for read. c) Documenting any recommendations made to enhance radiology services at Pine Ridge. C-5 Specific Exclusions of Responsibility The contractor will not be required to establish or maintain hardware at Pine Ridge designed to interpret films. C-6 Compensation The contractor will be paid a fixed flat rate per hour. No direct patient billing will be done by the contractor. The contractor will not do third party billing on its own for any interpretations done under this contract. C-7 Terms of Agreement and Termination The Government reserves the right to accept or reject services, if the level of work is unsatisfactory. 1. The Contract Radiologist shall demonstrate evidence of continuing education activities to acquire new skills, procedures, or qualifications which contribute positively toward the quality of service provided and achievement of contract obligations. 2. When requested, the Contract Radiologist shall participate in Service Unit quality assurance and peer review activities. 3. While at the service unit, the Contract Radiologist shall gain an understanding of the service unit health and safety requirements. 4. The Contract Radiologist shall treat patients with respect and show consideration for their personal privacy, maintenance of dignity and independence, social and cultural needs, and religious values and beliefs. The Contract Radiologist shall maintain a partnership with patients and practice in the context of family and community. The services to be performed by the Contract Radiologist shall be performed in accordance with Indian Health Service policies and procedures and the regulations of the medical staff-by-laws of the Pine Ridge IHS Hospital. The Contract Radiologist shall comply with all IHS facility infection control and safety procedures, practices, and standards. The Contract Radiologist shall meet all applicable Joint Commission on Accreditation of Healthcare Organizations (JCAHO) and Centers for Medicare/Medicaid Services (CMS) standards during the performance of this contract. The Contract Radiologists shall respect and maintain the basic rights of patients to include but not be limited to personal dignity. Applicable Standards and Contractor Requirements 1. The standards of medical practice and duties of the Contract Radiologist shall be determined pursuant to the bylaws of the Pine Ridge IHS Hospital, the regulations of the hospital, applicable provisions of law, other rules and regulations of any and all governmental authorities relating to licensure and regulation of physicians and hospitals, and the standards and recommendations of the Joint Commission on Accreditation of Healthcare Organizations. As appropriate, the Contract Radiologist shall participate in a quality improvement program consistent with current Joint Commission on Accreditation of Healthcare Organizations (JCAHO) requirements for ongoing monitoring and evaluation of the quality and appropriateness of care. 7. In accordance with the requirements of Federal Acquisition Regulation (FAR) 52.237-7, the Contract Radiologist shall provide evidence of insurability in the amount specified by the Contracting Officer prior to providing services at the Pine Ridge IHS Hospital. Upon request of the Contracting Officer, the Contractor shall furnish evidence of the insurability of the Radiologists performing services under this contract. The Contractor shall notify the Contracting Officer if it, or any of the Radiologists performing under this contract, change insurance providers during the performance period of this contract. The notification shall provide evidence that the Contractor and/or health-care providers will meet all the requirements of this clause including those concerning liability insurance and endorsements. The Contractor shall insert the substance of this clause in all subcontracts for Radiologist services under this contract. The Contractor shall be responsible for compliance by any subcontractor with the provisions set forth in this clause. 8. The Contractor shall comply with AAIHS Circular No. 95-01 (Dated 5/25/1995) on Medical Staff Credentialing. The circular establishes the elements of credentials review for current medical staff members and contractors working in area hospitals and health centers. Contractors shall provide sufficient verifiable credentials to warrant the granting of clinical privileges by the Pine Ridge IHS Hospital. The Contractors seeking clinical privileges shall request in writing the specific clinical privileges desired on the privileging document for approval by the Service Unit Governing Body. The Contract Radiologist shall not perform medical services until professional staff membership and clinical privileges have been granted in writing by the Pine Ridge Indian Health Service Unit Governing Body. As a prerequisite to performance under the resultant contract, Contract Radiologist performing the services must be privileged in accordance with the Aberdeen Area Indian Health Service credentialing policy. Therefore, compliance with the Indian Health Service privileging requirements is essential in the performance of the contract. Any failure to comply with IHS privileging requirements shall be considered to be nonperformance of the contract. The Governing body will not grant clinical privileges until it is determined that credentials information has been verified. Exceptions to the Indian Health Service credentialing review and clinical privileges process will not be considered. Any Contract Radiologist who fails to maintain medical staff appointment and clinical privileges will not be permitted to perform services under the contract and shall be promptly replaced by the Contractor. 9. The Contract Radiologist shall comply with the requirements of the Indian Child Protection and Family Violence Prevention Act (Public Law 101-630) and Section 231 of the Crime Control Act (Public Law 101-647). The service unit shall assist the contract providers in processing the necessary paperwork for conducting background Investigations on those providers who will have contact with Indian children in Indian Health Service facilities. Included in this requirement is a waiver provision that will permit certain contract providers to begin work prior to the completion of the full background investigation. 10. Upon the completion of the Indian Health Service Health Insurance Portability and Accountability Act (HIPAA) compliance plans, Contract Radiologists shall comply with the HIPAA compliance plan mandates particularly the requirements that relate to the protection of patient information. Each contract provider shall receive an orientation on specific responsibilities. 11. The Contract Radiologist shall have a Unique Physician Identification Number (UPIN) for billing purposes under Medicare Part B. Under Section 1861(r) of the Social Security Act of 1985, the Centers for Medicare and Medicaid Services (CMS) is required to establish a unique identifier for each provider who provides services for which payment is made under Medicare Part B billing. One UPIN is assigned to a provider regardless of the number of practice settings and the UPIN is assigned throughout the provider's Medicare affiliation in any state. The Contract Radiologist shall complete an "Authorization to Furnish Information and Assignment of Benefits" document to assign third party benefits to the Pine Ridge IHS Hospital. All third party payments resulting from the provision of Radiologist consultation services shall revert to the service unit. Contract Radiologist Qualifications Contract Radiologist providing services to patients of the Pine Ridge IHS Hospital must hold a current, unrestricted state medical license from any of the 50 United States, the District of Columbia, or the Commonwealth of Puerto Rico The contract Radiologist shall meet the same privileging requirements as permanent IHS providers before being allowed to provide medical services at the Pine Ridge IHS facility. The Contract Radiologist shall have a minimum of three years of meaningful patient contact and responsibility. Current Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certification is required for all contracted providers. The Contract Radiologist shall speak, understand, read, and write English fluently and communicate with sufficient clarity to be understood by Pine Ridge Indian Health Service Hospital personnel and patients. The Contract Radiologist shall possess sufficient initiative, interpersonal relationship skills and social sensitivity such that he/she can relate constructively to patients from the Native American community. The Contract Radiologist shall not be considered an IHS employee for any purpose. Completion of an approved medical residency-training program in radiology If applicable, a copy of the Education Commission of Foreign Medical Graduate certificate (ECFMG) of Fifth Pathway certificate. Verification from the contractor that the health status of the radiologist is sufficient for the performance of radiology services under the contract. Evidence of a recent physical examination shall be acceptable. Relevant experiences and training in radiology as evidence by three (3) letters of reference from individuals who can attest to the radiologist judgment, competence, and character, one letter must be from the chief of staff or department chairperson from a hospital where the applicant has provided radiology services. Provide information on previously successful or currently pending challenges licensure or registration (state or district, DEA) or the voluntary relinquishment of such licensure or registration; voluntary or involuntary termination of medical staff membership or voluntary or involuntary limitation reduction, or loss of clinical privileges at another hospital; and current involvement with professional liability actions All required certifications/ licensures in this resultant contract must remain current during the life of this contract The contract radiologist must not have any work or health restrictions which interfere with the performance of ultra sound services The contract radiologist shall maintain standards of personal hygiene & grooming compatible with the expectations of the Hospital clinic staff and the Indian community Contract Deliverables and Delivery Schedule The Contract Radiologist shall complete a Patient Care Component (PCC) document for each patient seen during the clinic day. The PCC shall be completed prior to the Contract Radiologist's departure from the service unit. The Contract Radiologist shall submit a workload report to the project officer within ten working days after the site visit. The report may be submitted via e-mail. The report may accompany the invoice and shall include:  A description of contract activities accomplished during the reporting period including the number of patients seen.  Any problems that have arisen or remain unresolved since the last report. The Contractor shall discuss the means to be taken to resolve pending problems.  Any recommendations and implementation plans resulting from any task. PROJECT OFFICER: The person identified below is Herby designated as the Project Officer for this contract. The project officer shall be responsible for : (1) Monitoring the Contractors technical progress, including surveillance and assessment of performance and recommending technical changes; (2) Interpreting the Statement of Work; (3) Technical evaluation as required; (4) Technical inspections and acceptance; and (5) Assisting the Contractor in the resolution of technical problems encountered during performance of this contract. Project Officer: Ted Cordier Pine Ridge, PHS, IHS Hospital 1201 E. Hwy 18 Pine Ridge, South Dakota 57770-1201 INVOICE SUBMISSION AND PAYMENT: The Contractor shall submit its original invoice to the Aberdeen Area Indian Health Service, Financial Management branch, 115 4th Avenue SE, Aberdeen SD 57401 and a copy to the Project Officer at the Pine Ridge, PHS, IHS South Dakota at the address listed above. The Contractor agrees to include the following information on each invoice. (1) Contractors name, address; (2) Contract Number (entire contract number must be included); (3) Invoice number and date; (4) Cost or price; (5) Dates of Service including the number of hours worked; and (6) Remit to address. The Aberdeen Area Financial Management Branch, 115 4th Avenue SE, Aberdeen, SD 57401, shall make payment. A. PROVISIONS AND CLAUSES: The following provisions and clauses apply to this acquisition. The FAR Provision 52.212-1 Instructions to Offerors Commercial Items, and 52.212-3 Offeror Representations and Certifications Commercial Items are incorporated by reference. The provision at 52.212-2 applies to this acquisition and is provided in full text. FAR 52.212-2 EVALUATION-COMMERCIAL ITEMS (JAN 1999): (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. THE CONTRACTOR SHALL PROVIDE EVIDENCE OF, OR SUBMIT A WRITTEN RESPONSE TO, THE FOLLOWING TECHNICAL EVALUATION FACTORS: The following factors shall be used to evaluate offers: Evaluation factors will be 60% technical factors and 40% cost factors for basis of award. B. 1. Availability, Qualifications, Education, Experience, & Competence; 40 points possible. A.. Availability (10 - Points) - The Offeror shall explain their understanding and agreement to provide seamless Pine Ridge IHS Hospital credential-able Radiology service to the Pine Ridge IHS Hospital in Pine Ridge, SD. Seamless being defined as "no break in Radiology service". The Offeror shall understand and explain the essential need for Pine Ridge Hospital to have a qualified and "Pine Ridge Hospital Credentialed Radiologist in the Pine Ridge Hospital, or on the immediate Pine Ridge Hospital housing suite) all of the time, AKA 8hrs/day,5days/week, throughout the entirety of this proposed contract. The Offeror shall understand and explain, in their technical proposal that Pine Ridge, SD is in an austere geographic region that many times encounters inclimate weather. Also, the closest airport is a ninety-(90) minute drive in good weather conditions. Further, the closest airport in Rapid City, SD possesses only three-(3) airlines that directly connect to four cities. Those cities being Las Vegas, NV, Denver, CO, Salt Lake City, UT, and Minneapolis, MN. Lastly, driving from the Rapid City Airport can sometimes be confusing, especially when driving at night. Planning, driving conditions, weather, and driving directions are essential when attempting to re leave a Radiologist when there rotation is ended. All points listed above in subfactor A.1. must be understood by the offeror in their owns words along with agreement to provide such "seamless" Radiology service when factoring the geographic and climatic considerations of Pine Ridge, SD and Pine Ridge IHS Hospital credentialing process. Failure to complete this will result in an unacceptable rating with zero weighted points. B. Qualifications & Education (10 - Points) - The offeror shall explain their understanding and agreement to provide Radiologists that meet educational and qualification credentialing requirements of Pine Ridge IHS Hospital. All Radiologists selected to provide Radiology services, as described in the request for contract proposal, shall have successfully graduated from an accredited school of Radiology. Also, all Radiologists shall have an active (ABR) American Board of Radiology certification or be (ABR) eligible. Additionally, all Radiologists shall possess malpractice insurance with at least a $1,000,000/$3,000,000 policy limit. The Offeror shall explain an understanding that any Radiologist provided for credentialing shall have a "risk-free" background check, or review. A history of prior drug or alcohol abuse shall disqualify any Radiologist applicant. Also, any practitioner databank review that reveals a repeated, or significant history of neglect, lack of competence, or behavior issues shall be disqualified. All Radiologists shall be required to provide a NPI number prior to be credentialed; Radiologists will also enroll for a Pine Ridge IHS Hospital Medicare provider identification number. The Offeror shall provide a statement of explanation acknowledging the above stated points in subfactor A.2. and assure the seamless delivery of Radiology service to Pine Ridge Hospital. Failure to complete this will result in an unacceptable rating with zero weighted points. C. Experience & Competence (10 Points) -The Offeror shall explain their understanding in their technical proposal that all Radiologists shall have at least one (1) full year of experience as a practicing Radiologist, documentation of time,when and where the Radiologist has provided solo independent Radiography services without any additional Radiologist backup. Additionally, each Radiologist the offeror provides shall be able to show competence in providing services including, but not limited to, general radiology, mammography,ct (computed tomography) procedures,flouroscopy, mri (magnetic resonance imaging)and ultrasound. The offeror must further explain the satisfactory comfort level of all Radiologists to perform these procedures on a Native American population with a high incidence of diabetes mellitus type II, hypertension, and history of cardiac disease, alcohol abuse and poor historians. Lastly, the offeror shall explain their understanding and ability to deliver seamless Radiology services as described in the above points in subfactor A.3. Failure to complete this will result in an unacceptable rating with zero weighted points. D. Qualified List (10 Points) - The offeror shall provide a list of actual Radiologists that meet the above points stated in subfactors A.1.2. 3. Note: Essential all Radiologists are competent at providing independent Radiology Services without any backup. Failure to complete this will result in an unacceptable rating with zero weighted points. 2. Managerial Ability to Achieve Delivery of Required Documents 40 points A. Knowledge of Required Documents (10 Points) - Proposals, which merely offer a statement to conduct/provide in accordance with the requirements of the Government Statement of Work, will not be eligible for award. Offerors must submit an explanation of the proposed technical approach in conjunction with the tasks outlined in the Statement of Work. The technical proposal shall provide a statement of the problem, scope, and purpose of the project in the offeror's own words to demonstrate a complete understanding of the intent and the Statement of Work requirements in the Request for Proposals (RFP). The offeror shall itemize and explain in the proposal all credentialing items that are MANDATORY for complete credentialing at Pine Ridge IHS Hospital. Also, the offeror shall itemize and explain in the proposal all required documents required by the credential coordinator BEFORE medical staff application can be processed. The offeror shall state in the proposal that all required documents required for medical staff membership and medical credentialing as stated in the work statement shall be in the office of the credentialing coordinator at least fourteen-(14) calendar days prior to arrival of any Radiology to Pine Ridge Hospital. Finally, all required documentation to be completed by a Radiology medical staff candidate shall legibly complete all sections of the credentialing and medical staff applications before processing can be initiated. Failure to recognize this in writing will result in zero points. B. Ability to Deliver Required Documents (10 Points) - The offeror shall explain in the proposal the understanding of the necessity of no more than three people working on the staffing documentation on the Pine Ridge Radiology account. Historically, lack of understanding of required documents has significantly delayed credentialing due to inconsistent information being passed on to Radiology candidates. Names of up to three personnel from the offeror that will be working on Radiology staffing for the Pine Ridge Radiology account shall be submitted in order to receive points in this technical subfactor. C. Managerial Accountability (20 Points) - The offeror shall list the one to three names of employees that will be assigned to the Pine Ridge Radiology contract (if awarded). Further, each of these one to three employees will sign a document citing all documentation required of Radiology in order to obtain medical staff application and medical credentialing privileges. Requirements as stated in the statement of work. Further, this document shall state all required documents will be required to be in the office of the credentialing coordinator at least fourteen-(14) calendar days prior to the Radiology candidate physically arriving to Pine Ridge Hospital. This document will be required in the technical proposal in order to receive the above twenty-(20) points. 3. Past Performance Information; 20 points A. (10 Points) - The technical proposal must include a listing of other Radiology Services contracts and subcontracts, Government and/or commercial, awarded to the offeror for the same or similar services. The contracts must contain the same type of requirements outlined in the Statement of Work. Include the following information for each contract and subcontract: a) Name of contracting activity. b) Contract Number. c) Contract type. d) Total contract value. e) Description of contract work. f) Contracting Officer and telephone number. g) Program manager and telephone number. h) Administrative Contracting Officer, if different from item f, and telephone number. i) List of major subcontractors. B. (10 Points) - The offeror must demonstrate its record of successful performance in past contracts and subcontracts, Government and/or commercial. Each offeror will be evaluated on its performance under existing and prior contracts for similar services. The offeror shall include the following past performance information: a) Identify the one contract, which you consider to be the most successful, and the reasons for this determination. b) Evidence of timely and acceptable contract performance on similar Radiology Services projects. c) Evidence indicating customer satisfaction for similar services outlined in the Statement of Work. d) Evidence of concern for cost control and budgetary considerations. TOTAL POINTS Possible...............................................100 points. Technical Criteria will be 60% and cost will be 40% determining factors for this award. (b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the options(s). (c) A written notice of award of acceptance or acceptance of an offer mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. The following FAR clauses cited in 52.212-5 are applicable to the acquisition: 52.203-6 with Alternate I, 52.219-6, 52.219-8, 52.219-28, 52.222-3, 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.225-13 and 52.232-33. Upon request, the Contracting Officer will provide full text copies of the FAR and HHSAR provisions and clauses. The provisions and clauses may also be accessed electronically at http://acquisition.gov/far/index.html and http://www.hhs.gov/oamp/policies/hssar.doc. In addition to business and technical proposal, Contractors shall provide the following: (see attachment) 1. Sign the SF-33 contract award form 2. Complete Offeror Representations & Certifications. If you are registered on ORCA (Online Representations and Certifications), please submit a copy with your proposal. 3. Complete and sign Licensure Requirement; provide a copy of current license. 4. Complete and sign Agreement to a temporary provisional of character investigations (for government contractors only). 5. Complete and sign the Declaration for Federal Employment - Optional Form 306. 6. Complete and sign Addendum to Declaration for Federal Employment (OF306). 7. Copy of malpractice insurance. PROPOSAL SUBMISSION INSTRUCTIONS: The Contractor shall provide evidence of, or submit a written response to, the technical evaluation factors in FAR 52.212-2. In addition, the Contractor shall submit a completed copy of FAR 52.212-3 with its offer. Contractors intending to conduct business with the Federal Government must register with the Department of Defense Central Contractor Registration (CCR) database prior to award. The CCR is the primary Government repository, which retains information on Government contractors. You may register via the Internet at http://www.ccr.gov or by calling (888) 227-2423 or (269) 961-5757. All responsible offerors may submit a proposal, which shall be considered by the Agency. Offers shall be submitted to the Aberdeen Area Indian Health Service, 115 4th Avenue SE, Room 309, Aberdeen, SD 57401, no later than 3 p.m. CST, on August 4, 2010. The offer must be submitted in a sealed envelope, addressed to this office, showing the time specified for receipt, the solicitation number, and your company's name and address. Offers will also be accepted by e-mail at lara.nelson@ihs.gov. ACCEPTANCE PERIOD: Your proposal must stipulate that it is predicated upon all the terms and conditions of this RFP. In addition, it must contain a statement to the effect that it is firm for a period of at least 60 days from the date of receipt by the Government.
