- FOR CLARIFICATION PURPOSES ONLY: Clarification is requested regarding Question 5 on GSA Form 1903, referencing GSA Form 3503. If contractors are unable to locate GSA Form 3503, or do not currently have a completed GSA Form 3503, they should ensure their Online Representations and Certification Application (ORCA) is complete and up to date at the time their proposal is submitted. Question 6 on GSA Form 1903, references supplements to SF-1442, what supplements to SF-1442 have been issued? No supplements to SF-1442 have been issued; however, contractors are required to submit all documents required per the solicitation and any subsequent amendment. Question 12 on GSA Form 1903, references appendixes to SF-1442, what appendixes have been issued for this solicitation? No appendixes have been issued to SF-1442; however, multiple amendments have been issued and all contractors are required to acknowledge receipt of each amendment according to the instructions in block 19 of the SF-1442 or by signing and returning each corresponding SF30 with their proposal
