- Attachment 3 to Amendment A00003 to HSFEMW-10-R-0028, Price and Cost Schedule Attachment 1 to Amendment A00003 to HSFEMW-10-R-0028, Statement of Work Attachment A to Amendment A00003 to HSFEMW-10-R-0028, Questions and Answers AMENDMENT # A00003 TO DHS, FEMA RFP # HSFEMW-10-R-0028 MWEOC Elevator Device Inspection, Testing, Maintenance and Repair Request for Proposal (RFP) No. HSFEMW-10-R-0028 is being amended to provide answers to questions submitted during, or after, the pre-proposal site visit held on Monday, 06-28-2010 at the DHS, FEMA, Mt. Weather Emergency Operations Center (MWEOC). The answers to these questions are provided as ATTACHMENT A to this Amendment # A00003. No further questions regarding any aspect of RFP No. HSFEMW-10-R-0028 will be accepted. To clarify issues addressed in ATTACHMENT A, a revised ATTACHMENT 1, STATEMENT OF WORK is provided as ATTACHMENT 1 to this Amendment # A00003. In addition, ATTACHMENT 3, PRICE/COST SCHEDULE has been revised and is provided as ATTACHMENT 3 to this Amendment # A00003. As stated in Amendment # A00002 to this RFP, the due date for submission of proposals is 4:00 PM EDT on Wednesday, 07-21-2010. Contractors shall submit an electronic proposal via email to Phyllis.Woodward@dhs.gov. The subject line of your email shall clearly reference RFP # HSFEMW-10-R-0028. Total size of email shall not exceed 8MB due to DHS email restrictions. Hand deliveries will not be accepted under any circumstances due to security concerns.
