Description
Evaluaion Criteria for DVD Encoding, Authoring,a nd Duplication Statement of Work for DVD Encoding, Authoring, and Duplication This is a Request for Quotation that is being set aside for small business concerns. You are invited to submit a quotation in response to this Request for Quotation (RFQ) titled "DVD Encoding, Authoring, and Duplication." The Centers for Medicare & Medicaid Services intends to award, under Simplified Acquisition Procedures (Dollar Threshold for Simplified Acquisitions $100,000.00) a firm fixed price purchase order. The contractor shall produce DVDs of training, outreach and education materials for Indian health care providers, staff and professionals in Indian Country and the American Indian and Alaska Natives eligible for Medicare, Medicaid and Children's Health Insurance Programs. The purchase order shall include production of DVDs for the Medicine Dish Volume 3 compilation of the 2009-2010 shows, basic Medicare and Medicaid training sessions, and informational programming to support CHIPRA grantees and others conducting outreach and enrollment activities targeting the AI/AN population. The NAICS Code is 334612. The attached Statement of Work and Deliverables provide the requirements the contractor is expected to accomplish in order to meet CMS's needs. The period of performance will be approximately August 9, 2010 thru August 8, 2011. Your response may be sent electronically in MS Word format to phillip.harrell@cms.hhs.gov or in paper no later than 12:00 pm on July 27, 2010 to Phillip Harrell, Department of Health and Human Services, Centers for Medicare and Medicaid Services, Office of Acquisitions and Grants Management, 7500 Security Blvd., C2-17-06, Baltimore, MD 21244. Responses shall be no more than six pages and shall include the following: (1) Contractor's capabilities overview; (2) Past performance information; and (3) Price of the deliverables. Your response will be evaluated on a best value basis using the following evaluation criteria: 1) Technical Approach 2) Past Performance and 3) Price. The Government intends to award an order to the responsible offeror whose quotation offers the best combination of technical capabilities and proposed price.
