Subcontracting Plan Subcontracting Plan Certification Client Letter Bank notification letter Business management questionsaire Statement of Work Solicitation Cover Letter The Federal Bureau of Prisons, Privatized Corrections Contracting, Office, Washington, D.C. is seeking quotes from sources having the ability to provide community-based outpatient Substance Abuse and Mental Health Services. Services must be within a five mile radius of the Spanish Fort Alabama city hall located at 7581 Spanish Fort Blvd., Spanish Fort, Alabama 36527 and should be accessible to local Public Transportation (if available). Performance periods under this contract will be for a one-year base period to begin on October 1, 2010 with four (4) one-year option renewal periods with service ending (if all options are renewed) on September 30, 2015. The services required for this contract include a Base Year, Option Year One, Option Year Two, Option Year Three and Option Year Four, respectively. The Following services are required and may be sub-contracted to another licensed contractor. The requirements for Transitional Drug Abuse Treatment and Mental Health Services for male and female Federal offenders includes: Base Year: Estimated number of inmates = 50 Intake Assessments (one per inmate) for 16 inmates = 16 assessments Individual Counseling Sessions ( 30 minutes = 1 unit) 48 units (per inmate) x 50 = 2,400 units Group Counseling Sessions (30 minutes = 1 unit) 72 units (per inmate) x 50 = 3,600 units Family Counseling Sessions (30 minutes = 1 unit) 20 units (per inmate) x 3 = 60 units Psychiatric Evaluation & Report = 1 report per inmate x 16 inmates = 16 reports Mental Health Intake Assessment and Report (one per inmate) for 10 inmates = 10 units Individual Mental Health Counseling (30 minutes =1 unit) 48 units per inmate for 12 inmates = 576 units Medication Monitoring (12 units per inmate) for 12 inmates = 144 monitoring units Option Year One: Estimated number of inmates = 52 Intake Assessments (one per inmate) for 18 inmates = 18 assessments Individual Counseling Sessions ( 30 minutes = 1 unit) 48 units (per inmate) x 52 = 2,496 units Group Counseling Sessions (30 minutes = 1 unit) 72 units (per inmate) x 52 = 3,744 units Family Counseling Sessions (30 minutes = 1 unit) 20 units (per inmate) x 5 = 100 units Psychiatric Evaluation & Report = 1 report per inmate x 18 inmates = 18 reports Mental Health Intake Assessment and Report (one per inmate) for 12 inmates = 12 units Individual Mental Health Counseling (30 minutes =1 unit) 48 units per inmate for 18 inmates = 864 units Medication Monitoring (12 units per inmate) for 18 inmates = 216 monitoring units Option Year Two: Estimated number of inmates = 55 Intake Assessments (one per inmate) for 20 inmates = 20 assessments Individual Counseling Sessions ( 30 minutes = 1 unit) 48 units (per inmate) x 55 = 2,640 units Group Counseling Sessions (30 minutes = 1 unit) 72 units (per inmate) x 55 = 3,960 units Family Counseling Sessions (30 minutes = 1 unit) 20 units (per inmate) x 7 = 140 units Psychiatric Evaluation & Report = 1 report per inmate x 20 inmates = 20 reports Mental Health Intake Assessment and Report (one per inmate) for 14 inmates = 14 units Individual Mental Health Counseling (30 minutes =1 unit) 48 units per inmate for 22 inmates = 1056 units Medication Monitoring (12 units per inmate) for 22 inmates = 264 monitoring units Option Year Three: Estimated number of inmates = 57 Intake Assessments (one per inmate) for 22 inmates = 22 assessments Individual Counseling Sessions ( 30 minutes = 1 unit) 48 units (per inmate) x 57 = 2,736 units Group Counseling Sessions (30 minutes = 1 unit) 72 units (per inmate) x 57 = 4,104 units Family Counseling Sessions (30 minutes = 1 unit) 20 units (per inmate) x 9 = 180 units Psychiatric Evaluation & Report = 1 report per inmate x 22 inmates = 22 reports Mental Health Intake Assessment and Report (one per inmate) for 16 inmates = 16 units Individual Mental Health Counseling (30 minutes =1 unit) 48 units per inmate for 24 inmates = 1152 units Medication Monitoring (12 units per inmate) for 24 inmates = 288 monitoring units Option Year Four: Estimated number of inmates = 55 Intake Assessments (one per inmate) for 24 inmates = 24 assessments Individual Counseling Sessions ( 30 minutes = 1 unit) 48 units (per inmate) x 59 = 2,832 units Group Counseling Sessions (30 minutes = 1 unit) 72 units (per inmate) x 59 = 4,248 units Family Counseling Sessions (30 minutes = 1 unit) 20 units (per inmate) x 11 = 220 units Psychiatric Evaluation & Report = 1 report per inmate x 24 inmates = 24 reports Mental Health Intake Assessment and Report (one per inmate) for 18 inmates = 18 units Individual Mental Health Counseling (30 minutes =1 unit) 48 units per inmate for 26 inmates = 1248 units Medication Monitoring (12 units per inmate) for 26 inmates = 312 monitoring units The services shall be in accordance with the Federal Bureau of Prisons Transitional Drug Abuse Treatment (TDAT) most recent Statement of Work dated April 2010. The request for quotation will be available on or after June 30, 2010 and will be distributed solely through the General Services Administrations Federal Business Opportunities web site http: //www.fedbizopps.gov. Hard copies of the quotation will not be available. The site provides downloading instructions. All future information about this acquisition, including quotation amendments, will be distributed solely through this site. Interested parties are responsible for monitoring this site to ensure that they have the most up-to-date information about this acquisition. The estimated closing date is on or about July 28, 2010. Faith-Based and Community -Based Organizations can submit offers/bids/quotations equally with other organizations for contracts for which they are eligible. This requirement is unrestricted. The BOP anticipates awarding a firm-fixed price contract. All responsible business concerns may submit a quote which will be considered by this agency.
