- The contractor is to provide Radiology and other Medical Imaging Services for the VAMC Louisville, KY for the VA Scott County Outpatient Clinic, 279 N. Gardner Street, Scottsburg, IN 47170. The services are to be performed at the Contractor's facility which must be located within a ten (10) mile radius of the Scott County VA Outpatient Clinic. The contract period will be for a base year and two (2) one-year options to renew. This is unrestricted commercial item procurement. The following commercial item clauses and provisions and any addendum apply: 52.212-1 Instructions to Offerors, 52.212-2 Evaluation, 52.212-3 Representations and Certifications, 52.212-4 Terms and Conditions and 52.212-5 Terms and Conditions to Implement Statutes and Executive Orders. NAICS: 621512. Solicitations will be issued o/a March 24, 2010. All questions must be in writing and may be emailed to john.gregorio@va.gov or faxed to 615-225-5432. Email is preferred. No telephone calls will be accepted nor messages returned. All amendments and other communications must be downloaded at FBO without further notification from the VA.
