Description
PERFORMANCE WORK STATEMENT (PWS) FOR FLIGHT FAMILIARIZATION TRAINING FOR THE UNITED STATES SCHOOL OF AEROSPACE MEDICINE (USAFSAM) AEROSPACE PHYSIOLOGIST OFFICER (APO) COURSE Dated: 15 April 2010 Revised 22 June 2010 1.0 INTRODUCTION: This effort includes tasks and activities to be performed by the contractor in support of USAFSAM. The contractor will provide flight instruction services required to meet the stated objectives and to accomplish the work required. The contractor will provide professional and technical qualified individuals to perform the tasks outlined in the following paragraphs in order to fulfill the requirements of the stated purchase order. The requirements are for introductory flight training and ground school as prescribed by the formal curriculum of the Aerospace Physiologist Course. 2.0 DESCRIPTION OF SERVICES: The contractor shall provide an instructor pilot cadre to provide direct technical instruction in the Diamond 20 or Cessna 172 aircraft under flight conditions prescribed in the curriculum. The contractor shall perform the tasks outlined in the following paragraphs to fulfill the requirements of this Task Order: 2.1 The contractor shall provide technical expertise and support on flight operations in the Diamond 20 or Cessna 172 aircraft to demonstrate flight conditions, aircraft handling and physiological effects of flight. The contractor shall provide technical and analytic support in a ground school module to include ground safety, preflight operations, administration of flight operations and airfield orientation. 2.1.1 Contractor shall provide a total of at least 8 hours of instruction for thirteen students in the Diamond 20 or Cessna 172 aircraft under the following conditions; introduction to aircraft handling, ground operations, taxi, take-off and landing, low level flight, introduction to instrument flight, spatial orientation and night operations. Overtime will not be authorized. 2.1.2 Contractor shall provide a detail schedule of proposed flight activities and lessons prior to course start date and will consult with course director on the details of the schedule. 2.1.3 Contractor shall provide licensed, knowledgeable and competent instructors in operating the aircraft employed during instruction. 2.1.4 Contractor shall coordinate scheduling for flight and ground school sessions. 2.1.5 Contractor shall accomplish student administrative requirements and maintain a log of student flight hours and attendance. 3.0 SERVICE SUMMARY (SS) Performance Objective PWS Paragraph Performance Threshold Surveillance Method/ Frequency Provide proper support on flight operations to demonstrate flight conditions, aircraft handling and physiological effects of flight. 2.1. Complete requested tasks with no more than two student complaints per week. Customer feedback/ Monthly Weekly and final progress reports on students enrolled in the flight training program. 7.0. Complete requested tasks within one day beyond objective. Customer feedback/ Monthly 4.0 GENERAL INFORMANCE 4.1 Period of Performance: 6 weeks 4.2 Place of Performance: Stinson Field, San Antonio TX. The contractor shall perform the above tasks within the airfield and other authorized flight environments. Some activities may take place in other normal office environments within the airfield complex while other activities will require flight activities outside Stinson Field. 4.3 RESPONSIBILITIES: The Contracting Officer Representative (COR) will be responsible for the tasking, technical liaison, review, and final acceptance of efforts to be accomplished under this purchase order. The point of contact for this effort will be Maj Ryan Maresh at DSN 240-3365, email: ryan.maresh@brooks.af.mil 5.0 GOVERNMENT FURNISHED PROPERTY: There are no Government supplies or facilities available to support this project. The contractor shall ensure classroom, materials and aircraft are provided for student instruction. 6.0 SECURITY REQUIREMENTS: There are no security requirements for this phase of training. 7.0 DATA REPORTING REQUIREMENTS: The contractor shall prepare and submit a weekly and final summary of student progress to capture total flight hours per student and ground school hours. The summary report will be prepared in accordance with contractor's format and shall also include status of funds spent to date and any student flight concerns/issues if applicable. All reports shall be forwarded to the Contracting Officer Representative (COR). 8.0 CONTINUATION OF ESSENTIAL DOD CONTRACTOR SERVICES DURING CRISIS: The performance of these services is not considered to be mission essential during time of crisis. 9.0 APPLICABLE APPENDICES: Not applicable.
