- Description
- The Centers for Disease Control and Prevention (CDC)Office of Surveillance, Epidemiology, and Laboratory Service (OSELS)Public Health Surveillance Program Office (PHSPO) intends to issue a sole source purchase order to Comeau Associates, 177 Ball Hill Rd., Milford, NH 03055-3409. The vendor will perform the following tasks: 1) Task I Development of 2011 Supervisor's Handbook Development and Dissemination, 2) Task II CATI Software updates, 3) Task III On- site training and/or telephone/email consultation for BRFSS Grantees for changes or upgrades, 4) Task IV Instruction and Troubleshooting for Monthly Data Submission, and 5) Task V On site Training. Comeau Associates is the only contractor that can perform all the requirements of this task because they are the author of programs and procedures that customize Ci3 software code and perform tasks essential to BRFSS survey purposes. No other person has legal access to the source code. This notice of intent is not a request for competitive proposals, however, any firm that believes it is capable of meeting, or exceeding the requirement as stated herein may submit a capabilities statement detailing their capability to include past performance information, which will be considered only if received by response deadline contained in this notice. Email capability statements by the deadline to Priscilla B. Turner at pgt5@cdc.gov. A determination by the Government not to compete this proposed contract based upon responses to this notice is solely within the discretion of the Government. Information received will be considered solely for the purposes of determining whether to conduct a competitive procurement.
