Description
This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in subpart 12.6, in conjunction with the policies and procedures for solicitation, evaluation, and award as prescribed under FAR 13.1, and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a written solicitation will not be issued. The proposed contract action is for a Sorin 3T Heater/Cooler Unit for a Heart/Lung Bypass machine or equal for the VA Medical Center Minneapolis, MN 55417. This brand name or equal piece of equipment must meet the minimum requirements as described in Section (V). (ii) The solicitation number is VA263-10-RQ-0271 and is issued as a Request For Quotation (RFQ). (iii) The provisions and clauses incorporated into this solicitation document are those in effect through Federal Acquisition Circular 2005-42, June 16, 2010. Provisions and clauses incorporated by reference have the same force and effect as if they were given in full text. The full text of the Federal Acquisition Regulations (FAR) and Veterans Affairs Acquisition Regulations supplement (VAAR) can be accessed on the Internet at http://www.arnet.gov/far (FAR) and http://vaww.appc1.va.gov/oamm/vaar (VAAR). (iv) This solicitation is issued as full and open competition. The North American Industry Classification System (NAICS) code is 339112, and the size standard is 29 million. (v) This requirement consists of two (2) line items: CLIN (1): Heater-Cooler System 3T (110V-60Hz) for Heart/Lung By-Pass Machine, Sorin Catalog number 16-02-85, Quantity: 3. CLIN (2): Bio-Med Training for 1 person (excluding transportation) Quantity 1. (vi) The Heater-Cooler System will provide 3 circuits for fast and efficient heating and cooling to meet patient cardioplegia temperature needs. Compressor based cooling and 3 independent water tanks eliminate the need for Operating Room water supply and ice. The system should possess the following characteristics: Remote control capability allows for quick temperature changes on all 3 circuits while the water evacuation feature ensures a spill proof clean up at the end of a case. The system includes 3 separate water tanks and 3 water circuits that can be used separately and simultaneously. Systems need to have: Extremely short pre-cooling times, Separate warm and cold cardioplegia tanks meet on demand changing temperature needs, Fast and efficient compressor based cooling, Independent safety system that prevents water temperatures from reaching critical values, Two-chamber pumps minimize any pressure inside the heat exchanger, and Water evacuation feature enables emptying of the heat exchanger and tubing for a spill proof clean up. System needs to be able to fit in specific dimensions and not be larger than 20in. x 34in. x 27in. and not weigh more than 102kg (228.5lbs.). Systems noise level operation needs to be less than 56dB (A). Must be US Manufactured. One operator and service manual included. Warranty of 12 months parts and labor. The price of all 3 systems must include FOB Destination. Offers must state standard warranty provisions for the systems. Offers must include training to one Bio-Med person, (excluding transportation, travel, and accommodations). The offer shall specify technical specifications and proposed adaptive equipment and provide estimated number of days to deliver. (vii) Delivery FOB Destination to VA Medical Center, One Veterans Drive, Minneapolis, MN 55417. (viii) 52.212-1 Instructions to Offerors - commercial Items, apply to this solicitation with the following addenda, VAAR 852.233-70 Protest Content; VAAR 852.236-76 Correspondence; VAAR 852.237-70 Contractor Responsibilities. (ix) 52.212-2, Evaluation - Commercial Items: FAR provision 52.212-2 does not apply to this solicitation. The Government will award a firm fixed price contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. (x) 52.212-3 Offeror Representations and Certifications - Commercial Items- Offerors are to include a COMPLETED COPY OF PROVISION 52.212-3 WITH THE QUOTE, a copy of the provision may be attained from http://www.arnet.gov/far. (xi) 52.212-4 Contract Terms and Conditions - Commercial Items: apply to this solicitation with the following addenda; VAAR 852.203-70 Commercial Advertising, VAAR 852.211-70 Service Data Manual, VAAR 852.237-76 Electronic Invoice Submission, VAAR 852.252.70 Solicitation Provisions or Clauses Incorporated by Reference. (xii) 52.212-5 Contracts Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items; The following FAR clauses identified at paragraph b of FAR 52.212.5 are considered checked and are applicable to this acquisition: 52.203-6, 52.219-28, 52.222-3, 52..222-19, 52.222-21; 52.222-26; 52.222-35, 52.222-36, 52.222-37,52.223-15, 52.225-1, 52.225-13; 52.232-34. (xiii) To be awarded this contract, the offeror must be registered in the CCR. CCR information may be found at http://www.ccr.gov. Warranty is to be 12 months parts and labor.
