- This is a combined synopsis/solicitation for commercial supplies and services prepared in accordance with the format in Subpart 12.6, and Far Part 13 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation is being issued as Request for Quote#VA-246-10-RQ-0483. The solicitation document and incorporated provisions and clauses are those in effect through the most recent Federal Acquisition Circular. The NAICS Code is 339112. The Department of Veterans Affairs Medical Center, Salem, Virginia has a requirement for a BAXA ExactaMix 2400 TPN Compounder System and intends to solicit quotes and award a firm fixed price contract. The contractor shall provide one BAXA ExactaMix 2400 Compounder System which consists of the following: (1) Main Compounder Module(live production unit;(1)Main Compounder Module(provided as backup only);(1)Display Module(live production unit);(1)Display/Module(provided as a back up only);(1)Load Cell Module(live production unit);(1)Load Cell Module(provided as back up only);(1)USB Laser barcode Reader(live production barcode reader);(1)USB Laser Barcode Reader(provided as a back up only);(1)Base Plate Module;(1)Vial Rack Module;(1)Calibration weight;(1)USB Keyboard;(1) USB Mouse;(1) USB Floppy Drive;(1) UPS Power Supply;(1) Laser Jet Printer;(1) Abacus SE (Single-Workstation Edition) Software. The following provisions apply to this solicitation: 52.211-6 Brand Name or Equal, 52.212-1, Instructions to Offerors-Commercial, 52.212-4, Contract Terms and Conditions-Commercial, Commercial Advertising, 852.271-70, 52.212-5, Contract Terms and Conditions required to Implement Statutes or Executive Orders-Commercial, including clauses incorporated by reference under Section b (1)(5)(11-16(23-24). Bidders are to submit pricing. Technical information is required if the item quoted is not BAXA. Process of determining contract award: award will be to the responsive, responsible bidder who can provide the VAXA product or a product that meets or exceeds the BAXA ExactaMix 2400 Compounder System specifications. Information on an alternative product should be specific, concise, and complete and provide sufficient information to demonstrate the products capacity to meet or exceed the requirements. The information required in provisions 52.212-1,52.212-2 and a completed copy of provisions 52.212-3, Offeror Representations and Certifications are to be provided with the quote.
