Description
This is not a solicitation announcement and should not be construed as such. This is a sources sought synopsis only. The purpose of this synopsis is to gain knowledge of potential qualified sources and their size classification (Hubzone, 8(a), Small Business, Small Disadvantage, Small Veteran Owned Business or Service Disabled Veteran Owned Small Business) relative to NAICS 339115 (Size Standard of 500 employees). Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. After review of the responses to this sources sought synopsis, a solicitation announcement may be published at www.fbo.gov website. Responses to this sources sought synopsis are not considered adequate responses to the solicitation announcement. All interested offerors will have to respond to the solicitation announcement in addition to responding to this sources sought announcement. The Veterans Integrated Service Network (VISN) 11 fabricates eyeglasses for the Indianapolis, Illiana HCS, NIHCS, Ann Arbor, Detroit VA Medical Centers and the Toledo OPC. It is necessary to have in place on site dispensing services to assist the Veterans of VISN 11. These on site dispensaries will serve various functions such as fitting, dispensing, patient education, and eye glass sales. To create a professional environment in which the Veterans will receive quality products and services. Contractor shall be qualified to educate and assist the Veterans in the proper choice, use and care of their eyewear. Appropriate state licensure/certifications shall be required if deemed necessary by the state. Contractor shall provide appropriate professional business license within the states required per each facility serviced. Contractor shall also comply with any other federal, state, and local law applicable to the industry. This documentation shall be provided annually during the life of the contract. The Contractor shall be responsible for furniture and equipment needed for operation. In addition to frame selection, the Contractor shall be responsible for all measurements and fitting of eyewear. Contractor shall fit Veterans with proper frame style for Veteran and prescription. Contractor shall determine the P.D., Seg height and/or optical center (OC) height. Contractor shall troubleshoot any optical issues before sending the Veteran back to the eye clinic Contractor shall fit and adjust glasses for Veterans. Contractor shall perform minor repairs on glasses, i.e. nose pads, screws and adjustments. Contractor shall educate Veterans on the advantages and disadvantages of different segment styles. Contractor shall fax orders to the VISN 11/12 Optical Laboratory Contractor may not batch orders for more than one day. Orders shall be faxed daily. Every faxed order shall include a cover sheet, location and account number. Contractor shall be required to obtain access to the VHA Computerized Patient Record System and enter patient measurements into the system. Any needed background/security checks shall be at the expense of the Contractor. Contractor shall provide completed eyewear for sale to individuals not entitled to eyeglasses, family members, and/or hospital staff. Contractor awarded this contract shall enter into a contract with the Veteran Canteen Service for sale of goods on government property. This contract will include a 12% commission payment on retail sales to the Veteran Canteen Service. Packing slips shall be included with each delivery for verification. If discrepancies are found, the Contractor shall notify the VISN 11/12 optical Lab within 24 hours. Contractor shall notify the Veteran when glasses are ready for pick up. Contractor shall fit and make adjustments on site or mail glasses to the Veterans home if the Veteran wishes it. Contractor shall provide the local Prosthetic service with a copy of the VISN 11/12 Optical Lab invoice and an invoice for dispensing fees. The invoice shall be typed and list the patient name with the last four of their social security number. This shall be done for each individual pair dispensed. Contractor shall be required to utilize an on-line order entry and/or database, which will help track patient service. Contractor shall responsible for including a packing slip with shipments sent to the VISN 11/12 Optical Lab. All shipments shall be sent via a carrier that can be tracked. Contractor shall ensure that all information needed to fabricate the eyewear is included on the order form sent to the VISN 11/12 Optical Lab. Potential sources must demonstrate experience and knowledge of required services and capability to provide such services. The purpose of this notice is to gain knowledge of potential qualified sources and their capabilities. Potential sources should identify business name, point of contact, business size, and any applicable business socio-economic category, i.e. Service Disabled Veteran Owned (SDVO), HUBZone, 8(a), Small Disadvantaged Business (SDB), Woman Owned (WO), and Veteran Owned (VO), service capability statement, provide information for recent and relevant projects similar in nature, DUNS number, address and any additional information felt to be pertinent. The NAICS code for this procurement is 339115 and the small business size standard of 500 employees. Responses to this sources sought is not a request to be added to a prospective bidders list or to receive a copy of the solicitation. THE VA IS NOT SEEKING PRICING OR OFFERS OF ANY KIND. There is no guarantee, expressed or implicit, that the market research for this acquisition will result in a particular set-aside or sole source award, or any other guarantee of award strategy.
