- Department of Labor Wage Determination Terms and Conditions Statement of Work This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 of the Federal Acquisition Regulation (FAR), as supplemented with additional information attached to this notice. This announcement constitutes the only solicitation; quotations are being requested at this time. A written solicitation will not be issued. The solicitation document and incorporated clauses and provisions are those in effect through the Federal Acquisition Circular 05-41. This combined synopsis/solicitation is issued as Request for Quotation (RFQ) # SSA-RFQ-10-1527. The Social Security Administration is seeking quotations for the purchase and installation of key switches. The specifications for the key switches and installations are included in Attachment 1 of this notice. The small business size standard is $14 million. The North America Industry Classification System (NAICS) code for this acquisition is 238210. Brand name or equivalent will be accepted. If you are quoting on an equal you must include the model or part number, picture and description with your quote. This acquisition is being conducted in accordance with the Federal Acquisition Regulation (FAR) Part 12, Acquisition of Commercial Items, and FAR Part 13, Simplified Acquisition Procedures. The selected offeror must comply with the following commercial item terms and conditions as well as any addenda to these clauses. The FAR clauses and provisions that apply to this acquisition are FAR 52.212-1, Instructions to Offerors - Commercial Items, FAR 52.212-4, Contract Terms and Conditions - Commercial Items. FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items. In addition, offerors must submit a completed copy of the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items. Note: If your company is registered in ORCA (Online Representation and Certifications Application), please read 52.212-3 (b)(2) of the Offeror Representations and Certifications-Commercial Items and indicate any changes, if applicable. If no changes need to be made, please include a statement that your representations and certifications are current, complete and updated for these products. If your company is not in ORCA, please fill out 52.212-3 (c thru m) of the Offeror Representations and Certifications-Commercial Items. Additional applicable terms and conditions, and representations and certifications are contained in Attachment 2 of this notice. To be considered for award, quotes must meet the salient characteristics specified in Attachment 1 of this solicitation. The contractor shall also provide a firm-fixed price quote for the item provided under this solicitation and state the period of acceptance for their quotation. The Contracting Officer will evaluate offers on the basis of information furnished by the offeror. The Contracting Officer is not responsible for locating or obtaining any information not identified in the quote. The Government will award a contract resulting from this solicitation to the responsible offeror whose quote conforms to the requirements of this solicitation in terms of technical acceptability, lowest price and delivery time. Shipping/Delivery must be free on board (FOB) destination CONUS (Continental U.S.) and installation shall be completed within 30 days after award. This contract will be a Firm Fixed Price contract with the following Contract Line Items structure: Line Item # Description Quantity Unit 0001 Hardware and Installation 59 EA of key switches on the unsecured side of mechanical, electrical, and telephone closets in accordance with the Statement of Work (See Attachment 1) This requirement is set-aside 100% for small business. Only qualified sellers may submit quotes. Responses to this combined synopsis/solicitation are due in writing no later than 3:00 p.m.EST, Tuesday, June 22, 2010. Offerors may send responses via email to nichole.mccollum@ssa.gov. All questions regarding this RFQ must be submitted in writing to Nichole McCollum at the email address provided previously by June 17, 2010 at 12:00PM EST. NO PHONE CALLS WILL BE ACCEPTED. E-mail communications shall include company name, point of contact, e-mail address, phone and fax numbers. Please provide your most favorable price quote based on your Open Market Price-list. Please also provide a copy of you commercial price list with your quote. Offerors must be registered in the Central Contractor Registration (CCR) database before an award can be made to them. If the offeror is not registered in the CCR, it may do so through the CCR website at http://www.ccr.gov.
