Total solicitation package should include the combined synopsis/solicitation with four attachments. • Attach. #1 is the Statement of Work. • Attach. #2 is the Pricing sheet which should include all pricing for the interior, exterior and post-wash lubrication. • Attach. # 3 is the performance rating questionnaire which must be forwarded to your references and your references must return them directly to the base by fax or email to the attention of Robert Kuch robert.kuch@niagarafalls.af.mil • Attach. #4 is the Wage Determination #2006-2371 Rev. #10. If you are missing any one of the four attachments, please contact the undersigned. b. The evaluation will be made with past performance, significantly more important than price, changing the 2nd paragraph on page 2 to remove lowest price technically acceptable and changing it to read past performance significantly more important than price. c. Attachment #1 is 13 (not 16) pages long. d. Pricing sheet is changed and attached - new pricing sheet should be used to quote. e. Performance rating questionnaire is four (4) pages in length. Ensure that all four (4) pages are forwarded to your references. If you are missing any pages, please contact the undersigned. f. You will need to acknowledge this amendment on your quote or by email to be considered for award. g. This quote is extended until 1200N EST 21 June 2010.
