- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation;quotations are being requested and a written solicitation will not be issued. This solicitation RFQ number VA-263-10-RQ-0246 is issued as a request for quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-30. No telephone requests will be accepted. Only written requests received directly from the requestor are acceptable. It is the Offerors responsibility to ensure that the quote is received. The associated North American Industry Classification System (NAICS) code for the procurement is 423450, with a small business size standard of 100. Requesters must state their business size on each request. The Department of Veterans Affairs, Des Moines, IA, has a requirement for the following items: 1. Four (4) Ventilator Transport Package-Item #1062628-V60 Ventilator Transport Package includes: V60 ventilator with C-Flex and AVAPS software options, color touch screen, 6 hour Li-ion battery, graphics package, oxygen hose, quick reference card and power cord; Universal Cart, circuit support arm bracket, V60 mounting plate, oxygen tank manifold/mounting bracket and oxygen tank holders. 2. Deluxe V60 Training-Item #1061221 Note: Installation is not included. *We are requesting the trade-in of four (4) BiPap Vision #582059. Item numbers are Jordan Reses and provided for informational purposes. Seeking comparable or same transport system. Delivery shall occur within 60 calendar days of contract award, shall be delivered to the VA Medical Center, 3600 30th Street, Des Moines, IA 50310. The contractor must indicate if there are freight charges. The Government will award a contract resulting from this solicitation to the responsible offerors whose offer conforming to the solicitation will be most advantageous to the Government, which represents lowest evaluated cost or price, technically acceptable bid. Technical acceptability will be evaulated based on the function and operation of the item in accordance with the Government's specifications or Equal. Offerors shall submit descriptive literature, product samples, technical features, and warrant provisions. Price will be evaluated to determine its fairness, completeness and reasonableness as it relates to the items offered.
