- Attachment 8: Recent and Relevant Experience References Attachment 7: Site Visit Information Attachment 6: Five (5) Digital Pictures of Existing Blasting Booth Attachment 5: Floor Plan Attachment 4: Statement of Work Attachment 3: Request For Quotation Attachment 2: Addendum to 52.212-2 Attachment 1: List of Attachments and Addendum to 52.212-1 Description: (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested, and a written solicitation will not be issued. (ii) The solicitation number F3C2MG0083A001 is being issued as a request for quotation. (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-41. (iv) This acquisition is 100 percent set aside for small businesses. The North American Industry Classification System (NAICS) code is 333319 with a size standard not to exceed 500 employees. (v) CLIN 0001 Plastic Media Blast Booth, MFR: Pauli Systems, Model RAM Blast Room, Part Numer 510-000-70 with two 5' full floor rooms OR an approved "or-equal" as described in Attachment 4 - Statement of work (SOW) and Attachment 3 - Request For Quotation (RFQ). (vi) Place of delivery and installation is Bldg. 862, Dock 2 at Minot AFB, ND 58705 and is FOB destination. (vii) The provision 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition. See Attachment 1 - Addendum to 52.212-1 (viii) The provision 52.212-2, Evaluation-Commercial Items, applies to this procurement. See Attachment 2-Addendum to 52.212-2 (ix) The government advises Offerors to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. Offerors are also reminded to complete Offeror Representations and Certifications on-line at www.bpn.gov prior to submitting an offer. (x) Clause 52.212-4, Contract Terms and Conditions Commercial Items, applies to this acquisition. (xi) Clause 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items, applies to this acquisition and the additional FAR clauses cited in the clause are applicable: 52.219-6, 52.219-8, 52.219-28, 52.222-3, 19, 21, 26, 35, 36, 37, 50, and 52.232-33. (xii) The following clauses also apply to this acquisition: 52.203-3, 52.204-7, 52.204-9, 52.223-5, 52.225-13, 52.232-1, 52.232-8, 52.247-34,52.252-2, 52.253-1, 252.204-7003, 252.204-7004 Alt A, 252.212-7001, 252.223-7006, 252.225-7001, 252.232-7003, and 5352.201-9101, 5352.223-9001. Clause 52.219-6(c) is applicable to this acquisition. This clause states, "Agreement. For the set-aside portion of the acquisition, a small business concern submitting an offer in its own name shall furnish, in performing the contract, only end items manufactured or produced by small business concerns in the United States or its outlying areas." The provision, 252.212-7000, Offeror Representations and Certifications, also applies to this acquisition. FAR 52.211-6 - "Brand Name or Equal" is applicable. (xiii) There is no Defense Priorities and Allocations System (DPAS) rating applicable for this procurement. (xiv) FAR 52.237-1 - "Site Visit" is applicable. See Attachment 7 - Addendum to 52.237-1 for Site Visit instructions. (xv) All offerors must submit an offer by 3:00 pm Central Standard Time on 6 July 2010.
