- JUSTIFICATION & APPROVAL Federal Acquisition Regulation Part 13.501 Based on the following justification, the use of other than full and open competition for the proposed contractual action is hereby approved pursuant to authority of 41 U.S.C. 253(C)(1) under FAR 6.302-1(a)2 (Only one responsible source and no other supplies or services will satisfy agency requirements). Justification 1. Requisition Number (FAR 6.303-2): 2. Requesting Department (FAR 6.303-2):Telehealth/Education 3. Contracting Activity (FAR 6.303-2):John J. Pershing VAMC, Poplar Bluff, MO VISN 15 4. Identity of person(s) preparing justification (FAR 6.303-2): Ruth Wertenberger 5. A brief description of intended use or application (FAR 6.303-2): This requirement is for one each Sapphire Video Server 2 Channel, 146 GB System, delivery, installation and training. This system is to expand an existing system to provide Telephone Initiated Guided Response (TIGR) on-demand patient and staff interactive program for our Healthcare System at John J Pershing VAMC CBOC in Sikeston, MO. 6. Demonstration of Contractors Unique Qualifications (FAR 6.303-2): Telehealth Services is the provider of the current system used at the John J Pershing VAMC. The software and system design is unique to this provider. Due to the recent expansion and opening of a new CBOC, and the need to provide consistency and continuity of the system and services throughout our organization, the Sikeston CBOC needs a TIGR patient education video system. The other four CBOCs already have units identical to this in use. The medical center also has this unit. This program is proprietary and in order to function properly must be consistent with existing applications. Market research was previously conducted to purchase and install the existing system and ultimately TELEHEALTH was awarded the contract. There are no other sources that provide this proprietary service/system. (FAR 6.303-2) 7. Description of Efforts Made to Ensure a Variety of Potential Sources (FAR 6.303-2): This procurement is being synopsized as an Intent to Sole Source giving potential contractors the ability to review and submit information. 8. Determination of Fair and Reasonable Cost (FAR 6.303-2): This procurement is being made under an existing FSS contract. 9. Description of Market Survey (FAR 6.303-2): Market research was conducted to determine the system is required to be compatible to the existing system used throughout the facility. A check of GSA to determine the item is current and available through FSS and 10. Listing of Interested Sources (FAR 6.303-2): Telehealth Services of St. Peters, MO. 11. Contracting Officer Certification: The Contracting Officer has determined that the justification is accurate and complete to the best of the Contracting Officer ™s knowledge and belief.
