- Under Statutory Authority 41 USC 253 (c )(1) the Government anticipates entering into sole source acquisition negotiations for placement and services of eligible veterans into 12-nursing home facilities located at: Clearview North, 199 Home Rd, Juneau, WI 53039; Capitol Lakes Health Center, 333 W. Main St, Madison, WI 53703; Four Winds Mnr, 303 S. Jefferson St, Verona, WI 53593; Oakwood Village East Health and Rehab Center, 5833 American Parkway, Madison, WI 53718; Evansville Mnr, 470 Garfield Ave, Evansville, WI 53536; Nazareth Health and Rehab Center, 814 Jackson St, Stoughton, WI 53589; Mount Carmel Medical, 677 East State Street, Burlington, WI 53105; Lake Country Health and Rehabilitation Center, 2195 N. Summit Village Way, Oconomowoc, WI 53066; Rennes Health and Rehabilitation Weston, 4605 Valdres Springs Ct., Weston, WI 54476; Mendota Lutheran Home, 500 Sixth Street, Mendota, IL 61342; Menominee Care Center, 501 2nd Street, Menominee, MI 49858; Roubal Care and Rehabilitation Center, N306 Maple St., Stephenson, MI 49887. This acquisition is for VA Hospitals/Medical Centers in VISN 12. Basic Ordering Agreements will be set up for a base year with 4 one-year options. The above authority is cited and justification made based on the determination that the Government's minimum needs can best be satisfied by services available from one source in a specific area of responsibility. This action is for services which the Government intends to solicit and negotiate with only one source under FAR 6.302-1. This notice of intent is not a request for competitive proposals. A determination by the Government not to open the requirement to competition based upon responses to this notice is solely within the discretion of the Government. Information received as a result of the notice of intent will normally be considered solely for the purpose of determining whether to conduct a competitive acquisition.
