- Statement of Work This is a Small Business Set-Aside Request for Quotation prepared in accordance with FAR Part 13, as supplemented with additional information included with this notice. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; A SEPARATE WRITTEN SOLICITATION WILL NOT BE ISSUED. QUOTES ARE BEING REQUESTED. The Centers for Medicare and Medicaid Services intends to award using Simplified Acquisition Procedures, a fixed priced purchase order for the 11th Annual Conference for Improving Health Outcomes, Health Disparities, and Health Equity on a sole source basis to Eleanor Walker Ph.D., RN, 12800 Libertys Delight Drive, Unit 302, Bowie, Maryland 20720 in accordance with that attached statement of work and deliverables. The North American Industry Classification Code (NAICS) is 541720. All business proposals final price shall be submitted in whole dollars only, rounded to the nearest dollar.' Responses must be sent electronically in MS Word format to renee.wallaceabney@cms.hhs.gov no later than 2:00pm. The period of performance is one year.
