Abbreviated Justification and Approval Document FOR OTHER THAN FULL AND OPEN COMPETITION of Requirements Not Exceeding $550,000 ( IAW FAR 6.304) 1. Contracting Activity: VISN 15 Network 2. Description of Action: Requirement:1 New 0 Repeat Pricing:1 Firm-Fixed Price 0 Time & Materials0 Cost Name of Proposed Contractor: Carrier Commercial Service Street Address: 8029 Flint City, State, Zip: Lenexa, Kansas 66214 Phone: 913 894-5962 3. Description of Services: The estimated value of the proposed action is $ 60,000.00. Describe the services or supplies, including make & model number, to be acquired below. The proposed action is to provide factory refurbishment to OEM performance specifications utilizing factory parts and factory service technicians to repair the two (2) Carrier 30GXN Air Cooled Chiller (Model #30GXN301F-640FX) serving surgery department. Due to impact surgery chillers have to medical center's OR operations and level of care, performance of both units shall be factory certified and commissioned to ensure reliability and completeness of refurbishment including installation of current software and firmware upgrades. 4. Authority Cited: (Identify the statutory authority, FAR title and FAR citation permitting other than full and open competition. It may be one of the following, but other exceptions may be used.) SOLE SOURCE 0 (a) 10 USC 2304(c)(1) or 41 USC 253(c)(1); FAR 6.302-1: Only one responsible source and no other supplies or services will satisfy agency requirements OR 1 (b) 10 USC 2304(c)(2) or 41 USC 253(c)(2); FAR 6.302-2: Unusual and compelling urgency. 5. Reason for Authority Cited: (State why this is the only source that can provide the required supplies or services.) 0 (a) Services can only be provided from the original source as this is a follow-on requirement for the continued provision of highly specialized services. 0 (b) Award to any other source would result in substantial duplication of cost to the Government that is not expected to be recovered through competition. 1 (c) Services/Supplies are needed immediately to satisfy mission requirements. (d) Additional Information: (i) What harm will come to the Government if desired supplies/services are not provided on time? Safety and care of VA surgery patients is jeopardized as both primary surgery chillers are not functional and temporary chiller does NOT provide redundant chilled water capabilities as required per VHA regulations to maintain proper climate control in OR area. Refurbishing main primary chillers with OEM parts and performing factory performance start-up, certification, and commissioning is paramount to ensuring continued reliability and warranty of the chillers. (ii) What harm will come to the Government if desired contractor doesn't receive the award? To ensure safety and reliability of the primary surgery chillers and re-establish performance standards, OEM components installed by factory authorized trained technicians to facilitate Carrier factory performed start-up services and factory certification of operation of both refurbished units needed to ensure strict climatic control for OR suites. 6. Actions to Increase Competition: (State what actions will be taken to increase competition before subsequent acquisition of the supplies or services is required.) Market research to determine level of available factory trained service contractors locally and nationally and contractors that are factory trained and authorized to perform start-up and provide factory performance certification. 7. Market Research: (Describe the extent of the market research conducted to identify all qualified sources and the results thereof. "Market Research" is defined as those attempts you made to ascertain whether other qualified sources exist. Research of the market place may consist of written, telephonic, or world wide web inquiries.) Market research consisted of web research of Carrier chiller authorized repair contractors and contact of local Carrier factory representative to obtain list of factory authorized trained technicians and branch offices. 8.
