- DOCUMENTS NEEDED BY PROSPECTIVE OFFEROR This is a Contract Delivery Route Solicitation. The following information will be needed to complete forms that will be returned with your proposal; HCR # 797A5 Solicitation # 300-106-10 Date of Solicitation – 05/14/2010 Begin Contract Date – 07/01/2010 End Contract Date – 03/31/2016 For Mail Service Between City, St – Big Spring, TX City, St –Sterling City, TX Annual Hours – 2,753 Annual Miles – 48,764 Attachment labeled " Forms Contractor must Complete and Return" will need the above information and be fully completed by the contractor. Completed forms will need to be sent to the following address; SOUTHERN TCMT SOLICITATION NO. 300-106-10 225 N HUMPHREYS BLVD STE 4126 MEMPHIS, TN 38166-7071 Contractor must make sure to include the Solicitation number in the address. All documentation for proposal must be received by the above address no later than June 14, 2010 by 1500 pm. Provide a photocopy of your Social Security Card or IRS Form CP 575 (EIN) *REQUIRED* NO SOLICITATION PACKAGES WILL BE SENT FROM THE CONTRACTING FACILITY OR THE AREA OFFICE.
