- This is an 8(a) and small business Sources Sought notice. This is NOT a solicitation for proposals, proposal abstracts, or quotations. The purpose of this notice is to obtain information regarding: (1) the availability and capability of qualified 8(a) sources; and (2) their size classification relative to the North American Industry Classification System (NAICS) code for the proposed acquisition. Your responses to the information requested will assist the Government in determining the appropriate acquisition method, including whether a set-aside is possible. An organization that is not considered an 8(a) or small business under the applicable NAICS code should not submit a response to this notice. THIS IS STRICTLY MARKET RESEARCH. THE CENTERS FOR MEDICARE & MEDICAID SERVICES (CMS) WILL NOT ENTERTAIN QUESTIONS REGARDING THIS MARKET RESEARCH. Background: CMS is seeking to identify companies with demonstrated capabilities for potential award of a contract to provide expertise to CMS to assist in the design, planning, organizational program development, and implementation of a Center for Medicare and Medicaid Innovation within CMS. Section 3021 of Public Law 111-148, The Patient Protection and Affordable Care Act of 2010, provides for the establishment of a Center for Medicare and Medicaid Innovation (CMI) within the Centers for Medicare & Medicaid Services by January 1, 2011. The stated purpose of the CMI is to test innovative payment and service delivery models to bring about a reduction in Medicare and Medicaid program expenditures while preserving or enhancing quality of care. Research and development for the Medicare and Medicaid Programs involves the detection of health care delivery problems and issues, the discovery of new program ideas and concepts to solve these problems, the development and testing of programs based on these hypothesized solutions, the evaluation of trials of these new programs, and based on the findings, the formulation of new policy proposals to successfully implement new program initiatives within Medicare and Medicaid. More succinctly, the purpose of research and development is to find and define the problem, develop and test programmatic solutions, and recommend new policy initiatives and program designs to implement new programs to address the original problem. The CMI shall be the research and development arm for CMS. In this role, the CMI will have several broad functions. The CMI will be the major focal point for the identification of problem areas in health care delivery and identification and testing of new models to improve program performance. The former will require program evaluation and a focused attention to the observations, comments and suggestions of internal and external stake holders to quickly identify areas where potential problems exist and to conduct monitoring and program evaluation to fully define and assess the problem area. The latter will require continuous monitoring of national and international literature describing new models to improve access and service delivery, increase efficiency and reduce costs, improve quality measurement and outcome improvement and to engage expertise to determine how these models might be applied to improve Medicare and Medicaid program performance. Once problem areas or new models are identified, the CMI will search for and/or elicit knowledge pertinent to gaining a more thorough understanding of the problem or new model and to provide background information important to discover and develop of new program initiatives to change and improve health care delivery, quality and/or reduce costs. In this respect, the CMI will be the primary vehicle for amassing and assessing information pertinent to the development of new and innovative health care delivery programs from the relevant literature, other federal agencies, universities, private foundations, providers and provider interest groups, general and disease specific patient advocacy groups, and the public at large. The CMI also will elicit from the public and develop internally, proposals for new program initiatives that can be used to pursue basic research objectives or to design new research demonstrations. In assessing and selecting new ideas and innovative models, the CMI shall consult with representatives of relevant federal agencies, and clinical and analytical experts with expertise in medicine and health care management and the CMI shall use open door forums or other mechanisms to seek input from interested parties. The second major role of the CMI is to design, implement and evaluate Medicare and Medicaid demonstrations and pilot programs to test the feasibility, cost effectiveness and quality outcomes of new health care delivery models. To facilitate and expand its capability, CMI may establish and manage contractual arrangements with private entities to develop and operate external research laboratories. The CMI will oversee the spending of considerable funds through a large number of contractual and provider arrangements. This will require the establishment of special mechanisms of auditing and fiscal oversight that will assure that tasks and projects are completed as contracted and funds are expended as intended. Another key role of the CMI is to promote research and demonstration transparency by disseminating findings from literature reviews, basic research and program evaluations to inform law makers and interested parties from government, academia, the health care industry, beneficiaries and entitles, and the public at large about health care delivery issues, new innovative concepts, and demonstrations and pilot programs. Finally, the CMI will act to sift through the information and evaluative findings to develop new objectives for basic research and new research demonstrations and provide guidance for the formulation of new program policy proposals and their implementation within the Medicare and Medicaid Programs. The legislative language in Section 3021 of Public Law 111-148 provides guidance as to the focus of CMI efforts to test payment and service delivery models under Medicare and Medicaid, provides the authority to extend and expand the operation of successful models, and stipulates the requirements of regular reports to Congress on the progress made by the CMI. Purpose and Objectives The purpose of this effort is to provide consultative management expertise to assist CMS in the development and implementation of a Center for Medicare and Medicaid Innovation (CMI) within CMS. Consultation would be provided to CMS in two topic areas: 1) The processes used by public and private organizations to research, develop, test, and disseminate innovations; and 2) The cycle time involved in research, development, testing, and dissemination of innovations to identify ways to improve efficiency in the process. Project Requirements CMS requests responses from 8(a) and small business firms to the following issues in each topic area: Topic area 1: The processes used by public and private organizations to research, develop, test, and disseminate innovations. Using examples of past performance, please discuss your firm's expertise and ability to: • Provide an overview of the organizational theory and theoretical concepts and constructs pertinent to the purpose, structure and function of research and development programs in large organizations; • Provide a comprehensive international literature review of organizational research and articles pertaining to the structure and operation of research and development programs in public and private sector organizations aimed at identifying and testing innovations; • Establish contacts with officials in public and private sector organizations in both the U.S. and other countries that would enable your firm to provide first-hand knowledge of research and development programs and access to pertinent senior officials in government and private entities; • Produce analytic reports that demonstrate the ability of your firm to describe the structure, function and performance of successful public and private sector organizations, both in the U.S. and outside, that are engaged in research, development, testing, and dissemination of innovations and that provide important design considerations for the establishment of a CMI at CMS. • Identify and assess the use of external research laboratories in government and private sector organizations in the innovation process and their applicability to the work CMS will be doing to expand its research and development capabilities in the CMI. Topic area 2: The cycle time involved in research, development, testing, and dissemination of innovations to identify ways to improve efficiency in the process. Using examples of past performance, please discuss your firm's expertise and ability to: • Conduct evaluations of the business processes and production lines of large organizations in order to provide practical ideas and proposals for improving efficiency to effect a reduction in the time between identifying a problem, evaluating possible solutions, testing such solutions and then adopting the solution in CMS programs. • Monitor, assess and develop reports about production and organizational performance that would demonstrate the ability of your firm to assist CMS in monitoring the activities and achievements of a CMI and furnish annual reports evaluating the performance of the Center in meeting the goals of the legislation and needs of CMS. • Develop practical guidance to CMS to stream-line processes to facilitate the implementation of Medicare and Medicaid demonstrations and which reflects an in-depth knowledge of the Medicare and Medicaid Programs. • Develop practical guidance to CMS to improve program monitoring and evaluation methodologies that will lead to a reduction in time needed to provide accurate reports of project results. Anticipated Period of Performance: It is also anticipated that the award will be issued as a three-year base and two (2) one-year options for a total of 5 years.
