- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is a combined synopsis/solicitation for 2 Multiple Amputee Trauma Trainers, accessories (including blood, gravity buckets, 2 batteries and 1 battery charger), and 12 months maintenance prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation number W9127Q-10-T-0022 is being issued as a Request for Quotation, RFQ. The solicitation and incorporated provisions are in effect through Federal Acquisition Circular 2005-40. The associated North American Industrial Classification System (NAICS) is 339999 the Small Business Standard is 500 employees. This requirement is unrestricted. The resulting contract will be a Firm Fixed Price Contract. The destination is FOB. LI 1: The specifications are as follows for the 2 Multiple Amputee Trauma Trainer: Lower body trainer that includes- Velcro Uniform (1), Severed left leg (1), Boots (1 left, 1 right), Right leg flesh chunks (3), Erect, and flaccid penises and injured scrotum (1 ea. As listed), Batteries (2), Battery Charger (1), RC transmitter (1), Blood mix - 5 gallon (1), Gravity bucket with hose (1) The specifications are as follows for the accessories: Battery-Lithium Ion KGS-TFX-BT-1 qnty 4 Blood - Coagulated (1 Gal) KGS-TFX-BLP-1G qnty 4 Blood Liquid (powder mix) KGS-TFX-BL-5G qnty 6 Bucket-Gravity Blood Fill KGS-TFX-BLB-1 qnty 1 Storage Case -Lower KGS-TFX-TCL-1 qnty 2 Storage Case-Accessories KGS-TFX-TCA-1 qnty 2 The specifications are as follows for the 12 month maintenance: Premium Care Annual Maintenance, Quarterly Customer Site Visit/unit 4 visits annually When quoting submit product make, model, and specifications.
