- scope of work This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The U.S. Department of Health & Human Services, Indian Health Service, Chinle Comprehensive Healthcare Facility, Chinle, AZ has a requirement for Personal Services Contract Physician Services Family Practice to provide services for the Outpatient Department. The Request for Quote (RFQ) number is CCHCF-AB-10-004 and incorporates provisions and clauses in effect through Federal Acquisition Circular (FAC 2005-14). This acquisition is being solicited on an unrestricted basis. This acquisition will be issued using Simplified Acquisition Procedures (SAP), FAR Part 13. The North American Industry Classification System (NAICS) is 621111 with a Size Standard of $9.0 million dollars. The resultant contract will be firm fixed price. Quotes are being solicited for the following items: CLIN-001: Physician Services (see attached statement of work). 320 hours (estimated). The hourly rate proposed shall be regular rate; CLIN-002: Weeknight on-call (Beeper Fee) Flat Rate: (15 hours per night). 16 shifts (estimated) CLIN-003: Weekend on-call (flat Rate) Saturday, Sunday and Government Holidays. 24 hours per shift. 8 shifts (estimated). CLIN-004: Call back Hours, Overtime Hours: 50 hours (estimated). Hours are from: 8:00am to 5:30pm, Monday through Friday, 36-40 hours per week. Period of Performance: May 24, 2010 through September 30, 2010. (Offerors should propose an all-inclusive hourly-rate when submitting an offer). Award will be made on a best value trade-off process. The following Federal Acquisition Regulation (FAR) provisions apply to this acquisition: FAR 52.212-1, Instructions to offerors Commercial Items (Sep 2006); FAR 52.212-3, Offeror Representations and Certifications Commercial items (Nov 2006) with Alt I (Apr 2002), all offerors shall include with their offer a completed copy of FAR 52.212-3, Alt I. An addendum is incorporated into the solicitation to include FAR clauses as follows: FAR 52.247-34 F.O.B. Destination (NOV 1991). Proposal submitted via U.S. Postal Service shall be addressed to the DHHS/IHS-Chinle Comprehensive Healthcare Facility, PO Box PH, Chinle, AZ 86503. Offers being submitted via any other carrier (i.e. FedEx or UPS) shall be addressed to the DHHS/IHS-Chinle Comprehensive Healthcare Facility, Off HWY 191 & Hospital Drive Chinle, AZ 86503. As a minimum, offerors shall include with their signed proposal the following information: (1) RFQ number, (2) company name and address, (3) point of contact with telephone and facsimile numbers and e-mail address, (4) a completed copy of both FAR 52.212-3 Representations and Certifications Alt I-Commercial Items, (5) price (all inclusive rate). Offers that fail to furnish required representations or reject the terms and conditions of this solicitation may be excluded from consideration.
