- This is a Sources Sought Notice (not a presolicitation notice pursuant to FAR Part 5) to identify sources that can provide all personnel to perform on-site services at the Hurlburt Field, Florida, Medical Treatment Facility (MTF) and Eglin AFB, Florida MTF where applicable. Services shall cover the range of operation in the performance of assisting eligible medical beneficiaries with appointments, referrals, and registration. The contractor shall also provide necessary and accurate reports in accordance with hospital, Air Force, and Health Care regulations during the performance of their duties during normal business hours. Services shall cover the range of operation of the: CENTRALIZED APPOINTMENT CALL CENTER (CACC) REFERRAL MANAGEMENT CENTER (RMC) PRIMARY CARE MANAGER BY NAME DESK (PCMBND) TRICARE PLUS ENROLLMENT CENTER (TPEC) FAMILY HEALTH CLINIC APPOINTMENT DESK (FHCAD) FAMILY PRACTICE CLINIC APPOINTMENT DESK (FPCAD) PEDIATRIC CLINIC APPOINTMENT DESK (PCAD) INTERNAL MEDICINE CLINIC APPOINTMENT DESK (IMCAD) FLIGHT MEDICINE CLINIC APPOINTMENT DESK (FMCAD) This sources sought is to determine the capability of potential sources and the availability of commercial sources and services pursuant to FAR Part 10. Please refer to the attached Workload Estimate. This notice is for informational purposes only and does not constitute an Invitation for Bid or Request for Proposal, and is not to be construed as a commitment by the Government. Interested small business sources only should submit responses by email to heidi.frazier@eglin.af.mil. Responses should contain Company Name, Address, Point of Contact, Cage Code, Phone Number, Fax Number, Size of Business pursuant to NAICS code 561421, small business size standard is $7M. In addition to the size of business, indicate all classes of business that fit your firm: i.e. Small Disadvantaged, Veteran-Owned, Woman-Owned, etc. Your response should include a statement of capability to the Workload Estimate. Responses must be received no later than 5 May 2010 1600 CST. All prospective offerors are responsible for monitoring this site for the release of all other information pertaining to the synopsis and solicitation. APPENDIX A-WORKLOAD ESTIMATE EGLIN CACC- The CACC receives approximately, 28,000 incoming calls and makes 5,000 outgoing calls monthly, while making approximately 20,000 appointment transactions. RMC-New consults which historically amount to 4,000 a month, outgoing calls approximately 4,000 monthly, incoming calls of 2,000 monthly, walk in patients approximately 300 a month PCMBND-. The PCMBN enrollment / registration process estimates approximately 500 transactions. The Tricare Plus enrollment figures are estimated to be approximately 20 a month. FMCAD- The FMCAD estimates approximately 2,000 incoming calls monthly and estimates making 1,000 appointments (booking, canceling, changing and telephone consults). IMCAD- The IMCAD estimates approximately 2,000 incoming calls monthly and estimates making 1,000 appointments (booking, canceling, changing and telephone consults). PCAD- The PCAD estimates approximately 2,500 incoming calls monthly and estimates making 1,500 appointments (booking, changing, canceling and telephone consults FHCAD- The FHCAD estimates approximately 10,000 incoming calls monthly and estimates making 4,000 appointments (booking, changing, canceling and telephone consults). FPCAD-The FPCAD estimates approximately 7,000 incoming calls monthly and estimates making 4,500 appointments (booking, changing, canceling and telephone consults Hurlburt The CACC receives approximately, 12,000 incoming calls and makes 1,000 outgoing calls monthly, while making approximately 7,000 appointments. The PCMBN enrollment / registration process estimates approximately 600 monthly. The Tricare Plus enrollment figures are estimated to be approximately 20 a month. The RMC handles approximately 40 referrals per day, approximately 600 walk ins per month
