- This is a combined synopsis/solicitation for commercial supplies prepared in accordance with the format in Subpart 12.6 and Far Part 13 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation is being issued as Request for Quote # VA-246-10-RQ-0366. The solicitation document and incorporated provisions and clauses are those in effect through the most recent Federal Acquisition Circular. The NAICS Code is 325412. The Department of Hunter Holmes McGuire Veterans Affairs Medical Center, Richmond, VA has a requirement for Exactrac Intra-Fraction Snap Verification and intends to solicit quotes and award a firm fixed price contract. This is a set aside for Service Disabled Veteran Owned Small Businesses (SDVOSB), Veterans Owned Small Businesses may submit proposal and will be considered if no responsible (SDVOSB). Contractor shall provide the following equipment/supplies/Service: (1) Exactrac Intra-Fraction Snap Verification Item number 20822, Qty 01 EA. (2) IGRT Review and approval software item number 20823, Qty 01 EA. (3) IGRT Prep & Review Station/ Data Server/2nd ET License item number 49961, Qty 01 EA. (4) Service On-Site by Support Specialist/Technician (1 Day) item number 82993-01, Qty 02 EA. (5) Case Coverage/Clinical Training on Site (one Day Training). (6) One (1) year warranty starting 6 months after delivery or 3 months after install. The following provisions apply to this solicitation: 52.211-6 Brand Name or Equal, 52.212-1, Instructions to Offerors-Commercial, 52.212-4, Contract Terms and Conditions-Commercial, Commercial Advertising, 852.271-70, 52.212-5, Contract Terms and Conditions required to Implement Statutes or Executive Orders-Commercial, including clauses incorporated by reference under Section b (1)(5)(11-16)(23-24); Offerors are to submit pricing. Process of determining contract award: award will be to the lowest-priced responsive, responsible bidder who can provide the item and service. This information should be specific, concise, and complete and provide sufficient information to demonstrate the offeror's capacity to meet or exceed the requirements of the contract.
