Description
April 26, 2010 To: All Prospective Quoters From: Contracting, Department of Veterans Affairs, Lebanon VA Medical Center Subject: Request for Quote (RFQ) #:VA-244-10-RQ-0217 : Janitorial Services Lancaster CBOC The Department of Veterans Affairs, Contracting Office is issuing this RFQ to solicit GSA Schedule contract holders for the purpose of entering into a Delivery Order under the schedule contract. LVAMC will conduct this acquisition using Subpart 8.4 under the Federal Acquisition Regulation. This requirement is for the Resource Management Department. Quoters interested in this acquisition may participate by submitting a response in accordance with the following instructions. I. RFQ DUE DATE: All responses to this solicitation are due NLT 4:00 PM EST Friday, April 30, 2010; email is the preferred method of delivery. II. ADDRESS FOR RECEIPT OF QUOTATIONS: Quotations shall be submitted electronically to the following email address. Michael.Bermes@va.gov or faxed to 717-228-5936 III. REQUIREMENTS: Cleaning of 6000 sq ft of medical office space. The Contractor shall provide all management, supervision, labor, tools, materials, supplies, and equipment necessary to provide medical grade janitorial/housekeeping services, five days per week (except Federal Holidays as later described), in accordance with all terms and conditions herein. 1.0Work Schedule Requirements 1.1General Cleaning of Entrance and Reception areas: Daily services to include: spot cleaning of inside and outside of entrance doors to include pick up of trash and cigarette butts. Dust all furniture and display units. Organize all magazines and reading materials on table tops and properly position furniture. Vacuum all carpeted areas in all high traffic areas. Mop all hard surface floor areas. Weekly services to include: Vacuum all carpeted areas wall to wall. Semi-monthly services to include: Dust all high and low vertical and horizontal surfaces and corners not cleaned in the course of normal dusting and remove cob webs. Quarterly services to include: Dust blinds, hanging light fixtures, and ceiling vents. 1.2General Cleaning of Patient Use Areas, Offices, Hallways, Conference Room & Common Areas: Daily services to include: spot cleaning of doors, door frames, light switches & doorknobs. Empty all waste paper receptacles and take trash to a designated area with the exception of hazardous waste disposal. Properly position the furniture in all offices. Vacuum all carpeted areas in all high traffic areas. Mop all hard surface floor areas. Spot clean all internal partition glass free of smudges and fingerprints. Clean and sanitize drinking fountains and water coolers. Weekly services to include: Vacuum all carpeted areas wall to wall. Dust and clean all office furniture including file cabinets, desks, (if cleared of paperwork) credenzas, counter tops, display units and window ledges. Clean and sanitize all telephones. Semi-monthly services to include: Dust all high and low vertical and horizontal surfaces and corners not cleaned in the course of normal dusting and remove cob webs. Clean base of chairs and and/or table legs. Monthly services to include: Dust picture frames and wall hangings. Quarterly services to include: Dust blinds, hanging light fixtures, and ceiling vents. 1.3General Cleaning of Exam Rooms & Lab areas: Daily services to include: Thoroughly scrub and disinfect sinks & countertops. Clean and disinfect all treatment area surfaces and fixtures. Spot cleaning of doors, door frames, light switches & doorknobs. Empty all waste paper receptacles and take trash to a designated area with the exception of hazardous waste disposal. Properly position the furniture in all offices. Vacuum all carpeted areas in all high traffic areas. Mop all hard surface floor areas. Spot clean all internal partition glass free of smudges and fingerprints. Clean and sanitize drinking fountains and water coolers. Weekly services to include: Dust bases of exam room tables. Vacuum all carpeted areas wall to wall. Semi-monthly services to include: Dust all high and low vertical and horizontal surfaces and corners not cleaned in the course of normal dusting and remove cob webs. Quarterly services to include: Dust blinds, hanging light fixtures, and ceiling vents. 1.4General Cleaning of Kitchen, Break Room, Lunchroom & Coffee Area: Daily Service to include: Thoroughly scrub and disinfect sinks, countertops, tables, exterior of all appliances & inside of microwave. Spot clean the wall behind / adjacent to the trash receptacle. Spot cleaning of doors, door frames, light switches & doorknobs. Empty all waste paper receptacles and take trash to a designated area with the exception of hazardous waste disposal. Restock products such as paper towels, hand soap, liners, and deodorant products. Vacuum all carpeted areas in all high traffic areas. Mop all hard surface floor areas. Weekly services to include: Vacuum all carpeted areas wall to wall. Semi-monthly services to include: Dust all high and low vertical and horizontal surfaces and corners not cleaned in the course of normal dusting and remove cob webs. Quarterly services to include: Dust blinds, hanging light fixtures, and ceiling vents. 1.5General Cleaning of Restrooms: Daily Service to include: Clean all dispensers and fixtures. Clean and disinfect wash basins, toilet bowls, urinals, and counter tops. Spot clean toilet partitions. Spot cleaning of doors, door frames, light switches & doorknobs. Empty all waste paper receptacles and take trash to a designated area with the exception of hazardous waste disposal. Polish all metal and mirrors. Restock products such as paper towels, toilet tissue, hand soap, liners, and deodorant products. Mop all hard surface floors. Semi-monthly services to include: Dust all high and low vertical and horizontal surfaces and corners not cleaned in the course of normal dusting and remove cob webs. Quarterly services to include: Dust blinds, hanging light fixtures, and ceiling vents. 1.6Floor Maintenance Program: Quarterly services to include: Vinyl tile maintenance (1 time per year strip and refinish, 3 times per year scrub and recoat) on all tiles areas. Semi - Annual services to include: Hot water carpet extraction. Monthly services to include: Burnishing of all vinyl tile areas.
