Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplement with additional information included in this notice. This announcement constitutes the only solicitation to be sent out; proposals are being requested and a written solicitation will not be issued: Solicitation #CCHCF-10-0002: This combined synopsis/solicitation is issued as a Request for Quote. NAIHS CODE: 621210. Description: The requirement is for a firm-fixed price (FFP) type purchase Order. CLIN: 001-- The Contractor shall provide on-site Geriatric Dentistry Services as scheduled by Chinle Dental Department. (Fabricating/processing denture services). The Contractor shall provide all management, supervision, labor, materials, supplies, equipment, and transportation. Period of Performance: May 17, 2010 through September 30, 2010. Services will take an estimate of 4-5 trips to the Chinle Hospital to complete the services. Estimated Quantity of Dentures for the Geriatric Dentistry Services: 1. Full Dentures (Upper and Lower) - 45 EA 2. Full Partials - 7 Ea 3. Upper OR Lower Dentures - 17 EA 4. Upper OR Lower Partials - 16 Ea 5. Reline - 1 EA 6. Lab Fee - 1 Ea The rates shall be inclusive of all necessary and associated costs, (i.e. travel, per diem, lodging, rental car, applicable taxes, One lab assistant, etc.), to complete the task. Contractor shall perform the Following: 1. the doctor and the Lab Assistant shall come to Chinle Hospital for an estimate of 3 days for On-site, Chair side Patient procedures services to perform impressions, jaw relations and bite registration on edentulous and partially edentulous patients for the fabrication of full dentures, partial dentures and combinations thereof. 2. Materials/Equipment shall be provided by the Contractor. Facilities and Staff will be provided by the CCHCF Dental Program, except for the Contractor's laboratory technician. 3. Contractor shall perform the necessary laboratory procedures at their facility. 4. Contractor shall return to Chinle Hospital, couple of weeks later, for two days, to fit in the Prosthetic appliances to ascertain correct function, form, and esthetics, 5. Contractor shall return to Chinle Hospital, a couple of weeks later again to insert the prosthetic appliances. 6. On a return visit again, Final Visit, Contractor shall make any appropriate post-insertion adjustments. All full workdays will be from 8am to 5pm in the Dental Department of the Chinle Hospital. Total of 4-5 visits to Chinle Hospital, Chinle, AZ Interested firms are invited to respond by sending their proposals with the following: (1) Announcement number, (2) Company name, DUNS, EIN, address, telephone and fax number and (3) cost breakdown of proposal. IHS will not pay for any cost incurred associated with this Request for Proposal. Written information is due at the Chinle Comprehensive Health Care Facility, PO Drawer PH, Chinle, AZ. Award shall be made at a fair and reasonable price to the responsive and responsible offeror whose quote conforming to the requirements herein will be the most advantageous to the Government and whose quote represents the best value as defined by FAR 2.101. In determining best value, the following factors will be considered: past performance, Price and prior IHS experience. The Contractor should have training in Geriatric Dentistry and have expertise at fabricating full, upper, lower and partial dentures As part of the evaluation process, candidates may be interviewed to ensure their understanding of the scope of work and to verify their qualifications to perform required services. The socio-economic status of an offerer may also be considered, should one or more quotes represent the best value. Effective October 1, 2003, vendors must be registered in the Central Contractor Registration (CCR)) in order to receive contracts from the Federal Government pursuant to FAR Clause 52.204-7.
