- Grounds Maintenance soliciation package This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is CLA-10-R-0019 for grounds maintenance services. This solicitation is issued as a Request for Proposal (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-24. This acquisition is 100% set-aside for small businesses. The NAIC code is 561730 and the small business size standard is $6.5M. The FOB point is Claremore Indian Hospital, 101 S. Moore Avenue, Claremore, Oklahoma 74017. Evaluation Criteria is as follows: I. Method and Approach: The offeror's response shall provide method and approach to performing the grounds maintenance scope of work in accordance federal, state and local policies and procedures. (35 pts) II. Personnel, Insurance and Chemical qualifications: Provide list of chemicals in accordance with EPA guidelines, proof of certification and licenses and insurance (35 pts) III. Past Performance: The contractor must possess at least two years of grounds and lawn care experiences and 5 years in applying lawn chemicals. Submit 3 references with contact name, address and telephone number (30 pts) Evaluation criteria and price will be paramount. Offerors are to include a completed copy of the provision found at FAR 52.212-3, Offeror Representations and Certifications - Commercial Items with its offer. The clause found at FAR 52.212-4, Contract Terms and Conditions - Commercial Items applies to this acquisition. The clause found at FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items applies to this acquisition.
