- This is a modification to paragraphs: (1) one, (2) two, (3) three, and (5) five. All other paragraphs from the original pre-solicitation notice remain unchanged. Paragraph (1) one: 1. The Department of Veterans Affairs intends to solicit quotes for a Service Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), HUB Zone, Small Disadvantaged Business (8a), Woman Owned Small Business, Small Business, or other that is able to provide Radiation Therapy services to the Central California Healthcare System (VACCHS), Fresno, CA at the Contractor's facility. Offers from all capable and responsible sources will be considered. Contractors responding to this notice need to provide the below information to eric.toliver@va.gov no later than May 3, 2010. Paragraph (2) two: 2. Firms offering a response to this notice should ensure that they are registered in the Central Contractor Registration Database http://www.ccr.gov/ and ensure that the NAICS code 621111 is included in the CCR profile prior to submission of bids; is verified for ownership and control, and is so listed in the Vendor Information Pages database, http://www.VetBiz.gov, if applicable. Paragraph (3) three: 3. The anticipated period of performance is from June 1, 2010 to May 31, 2011 for a base period, plus one (1) one-year option period. Paragraph (4) four: 4. Telephonic requests for the solicitation will not be honored. When issued, the solicitation will be available via the FedBizOpps website, http://www.fbo.gov/. The tentative solicitation issue date is May 4, 2010 and the anticipated closing date is May 19, 2010. Paragraph (5) five: 5. Interested offerors are advised to consult the actual solicitation for final dates and times. A complete Statement of Work (SOW) will be included in the solicitation. The Request for Quotations (RFQ) number is VA-260-10-RQ-0210.
