Request for Information Number VA-791-10-RP-0011Assisted Living Pilot for Veterans with Traumatic Brain Injuries (AL-TBI) 1.0DESCRIPTION The VA Denver Acquisition and Logistics Center (VA DALC) in support of the Veterans Health Administration (VHA) is seeking information on the availability and interest of providers of long term residential rehabilitative care for Veteran patients with traumatic brain injuries (TBI). This Pilot Program will include the period from date of award through September 30, 2010, with options extending through September 30, 2013. Multiple indefinite delivery type contracts are planned to cover several locations within the continental United States. Interested providers are invited to review the attached Statement of Objectives (SOO) and provide comments along with capabilities and capacity to provide the desired services. Specifically, comments are encouraged in the following areas: 1.CARF Accreditation: Is your facility accredited by the Commission on Accreditation of Rehabilitation Facilities (CARF)? If not CARF accredited please explain why your facility is equivalent to one that is CARF accredited. 2.Which services described in the SOO does your facility normally provide? 3.How do you normally bill for the rehabilitation services described in the SOO? Is there a daily rate that covers all the services identified in the SOO? 4.Describe other information about your facility and services to help demonstrate your capacity and capabilities, including the location of the facility, number of beds, etc. 5.Indicate your size status per the SBA guidelines (see http://www.sba.gov/contractingopportunities/officials/size/index.html). 6.Other recommendations and comments are welcome. 2.0 THIS IS A REQUEST FOR INFORMATION (RFI) ONLY. This RFI is issued solely for information and planning purposes - it does not constitute a Request for Proposal (RFP) or a promise to issue an RFP in the future. This request for information does not commit the Government to contract for long term TBI residential rehabilitative care services. Further, the VA is not seeking proposals and will not accept unsolicited proposals for these services. Respondees are advised that the U.S. Government will not pay for any information or administrative costs incurred in response to this RFI; all costs associated with responding to this RFI will be solely at the interested party's expense. Not responding to this RFI does not preclude participation in any future RFP, if any is issued. If a solicitation is released, it will be synopsized on the www.fbo.gov. It is the responsibility of the potential offerors to monitor these sites for additional information pertaining to this requirement. 2.03.0 DUE DATE: Submittal of requested information is due not later than 4:00pm Mountain Standard Time, on May 12, 2010. If brochures are available then attach them to the email, or identify a website where the brochures can be downloaded, or mail six (6) sets to: Department of Veterans Affairs Denver Acquisition & Logistics Center Attn: Mike Sanford, Rm 501 155 Van Gordon St. Lakewood, CO 80228 4. 0 STATEMENT OF OBJECTIVES 4.1.Overall Objectives: The overall objective of the Assisted Living Pilot for Veterans with Traumatic Brain Injury (AL-TBI) program is to contract with suitable providers of long term residential rehabilitative care for Veteran patients with traumatic train injuries (TBI) and to assess the effectiveness of providing AL-TBI residential rehabilitation services to eligible Veterans. This includes several levels of care: a.Neuro-Rehabilitation Program - designed to provide the support and services required with the goal of improving the quality of life and returning the Veterans with TBI to independence and, when practical, to the home, community, work, and school. Individualized plans of care will be developed and written for each Veteran to address specific goals relating to medical, physical, cognitive, and emotional areas of concern. The plan of care will be based on a comprehensive evaluation of the physical, cognitive, vocational, and psychosocial needs of the individual. Treatment and support services will be provided to meet the specific rehabilitation and reintegration needs of the Veterans. b.Neuro-Behavioral Rehabilitation Program - primary focus on teaching behavioral alternatives to various unsafe or maladaptive behaviors. Behavioral programs generally use an interdisciplinary approach that may include behavior modification, medications, socialization skills training, substance abuse treatment, family therapy, and physical management programs, as well as traditional interdisciplinary treatment. The goal of the program is to enhance participation in daily routine and community activities and to enable the Veteran to do so independently. c.Supported Living Programs and Ongoing Support Program - designed to assist Veterans with TBI to live a full and productive life by enabling each participant to preserve the goals reached in earlier stages of rehabilitation and to sustain their preferred quality of life. Services may be provided in various living environments, including small group congregate homes, supervised apartments, in an individual's home with varying degrees of support, or in a day program setting. d.The full range of rehabilitation services should be available on-site or accessible in the community. These services include: "Physical Therapy "Speech Language Pathology "Occupational Therapy "Social Work "Psychology/Neuropsychology oCounseling oCognitive Therapy "Medication Management and medical services "Behavior management plans "Care management "Independent Living Skills "Vocational Counseling "Social and recreational activities "24-Hour security and staff availability "Transportation with Attendant Services "Cable, computer and internet access "Health promotion and exercise program "Access to gym and exercise facilities 4.2.0 Contract Objectives: The objective of this procurement is to result in multiple contracts with providers of suitable residential rehabilitative care that meet the objectives of this SOO. Offerors must submit a Performance Work Statement in response to the solicitation that demonstrates how they will meet the objectives of this SOO. Contract objectives include, but are not limited to, the following: a.Multiple contracts are anticipated with multiple contractors in order to create a broad facility base for the patients. b.Contracts will be of an Indefinite Delivery/Indefinite Quantity type because identification of the patient population is an ongoing process of the AL-TBI pilot program. c.Specific orders for care will be placed by authorized coordinators at individual VA Medical Centers in coordination with the AL-TBI Program Manager. d.Selection of a contractor for receipt of an order will be at the discretion of the VA Medical Center placing the order, the AL-TBI Case Manager, the AL-TBI Program Manager and the COTR. e.It is anticipated that the Pilot Program will continue through fiscal year 2011 with fiscal years 2012 and 2013 as option years. 4.3.0 Program Objectives: a.Contractors must furnish a care management system that will provide programmatic schedule and performance information to the Government throughout the AL-TBI performance. b.Establish a sound risk management system. Mitigate risk to the maximum extent possible, especially on software development efforts through integration of metrics to monitor program status. c.Establish a comprehensive communication management approach with the appropriate VA AL-TBI Case Manager and the AL-TBI Program Manager. 4.4.0 Specific Objectives: a.In addition to the levels of care outlined above the Contractor shall also provide the following services specified in the individualized plan of care for each resident: i.Laundry and housekeeping services; ii.Meals and snacks plus assistance in eating, as needed; iii.Medication oversight, as needed; iv.Access to emergency services, as needed; and v.Other services or supplies commensurate with the VA authorized level of care, without extra charge. b.Contractor shall maintain certification or approved status as a provider in the Medicaid and Medicare programs, licensed or approved and in good standing with State licensing bodies, and full accreditation by a national accreditation body such as the Commission on Accreditation of Rehabilitation Facilities (CARF), or VA approved CARF equivalent. c.The Quality Assurance Plan (QAP) will provide for routine measurement and analysis of clinical, fiscal and administrative program quality elements. Action plans are developed and implemented as necessary. The minimum quality elements that must be monitored include: i.Patient and/or family concerns ii.Annual verification that the Contractor remains in good standing with Federal and State licensing, certifying agencies, and national accrediting bodies. d.Upon award contractors must submit for VA approval their Safety and Sanitation practices that comply with the provisions of the National Fire Protective Association's NFPA 101, Life Safety Code (1994 edition) and the NFPA 101A Guide on Alternative Approaches to Life Safety (1995 edition). e.The VA, at its sole option, will monitor the professional care and fiscal and administrative management of services provided to VA beneficiaries under this contract, through one or any combination of the following methods; reviews of State agencies reports, site visits by a VA AL-TBI inspection team, telephone contact to ensure patients are receiving the services according to the Contractor's plan, and/or on-site monitoring of patient's care by the AL-TBI Case Manager and/or AL-TBI Program Manager. Patient and family satisfaction with these services will be assessed. It is agreed that the Contractor shall provide VA with copies of all State agency/other accrediting body reports, including reports of annual site surveys when requested. All medical records concerning the Veteran's care shall be readily accessible to VA without additional cost to VA. Upon discharge or death of the patient, medical records will be provided to the VA. f.It is agreed that the Contractor will collaborate and cooperate with VA medical center staff and the pilot program management staff in the conduct of the evaluation of the pilot program. g.The contractor shall provide the VA Contracting Officer's Technical Representative (COTR) annually or upon request: i.Detailed updates on changes to the program of services; ii.Methods to be used for monitoring ongoing education and evaluating staff competency to assure that staff is current with the requirements for professional education and training in neurorehabilitation care; iii.Updates on credentials and licensing for administrative and clinical staff; iv.Updated staffing plans; v.Documentation of current State license and report; vi.Documentation of certification as a Medicaid and Medicare program provider and report; vii.Verification of accreditation (CARF, JCAHO/other) and report; viii.Documentation on disaster preparedness and emergency management plans; ix.Changes to the Quality Assurance Plan (QAP); x.Cooperation with the VA evaluation team to facilitate assessment of patient/stakeholder satisfaction including internal patient/stakeholder satisfaction data; xi.Cooperation with VA evaluation team to provide data on facility characteristics and patient variables including demographics, clinical characteristics and health outcomes h.The contractor shall report any events, classified as "Reportable Events", including any significant change in the patient's medical, cognitive, and behavioral status under applicable State requirements, to the VA AL-TBI Case Manager and the AL-TBI Program Manager within 24 hours of becoming aware of the event. i.The contractor shall provide written documentation of the individualized plan of care to the AL-TBI Case Manager and the AL-TBI Program Manager every 30 days. j.Referral for traumatic brain injury care services shall be initiated by the Veteran's VA health care team.
