- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional informationincluded in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This acquisition is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-36. The North American Industry Classification System (NAICS) Code for this procurement is: 339112. This requirement is solicited unrestricted. The U.S. Veteran Affair, Medical Center, Boise Idaho has a requirement for following: Line Item 0001, Mfr: Karl Storz, Part Number: P-20040908U-KT, AIDA COMPACT, Quantity: 3 Line Item 0003, Mfr: Karl Storz, Part Number: 9423HDNBF, 23" Wideview Monitor, Quantity: 4 Line Item 0004, Mfr: Karl Storz, Part Number: 22220055-3, H3Z HD Camera Head, Quantity: 7 Line Item 0005, Mfr: Karl Storz, Part Number: 20330320-1KT, Endomat LC Kit Suction Pump and Tubing, Quantity: 1 Line Item 0006, To include removal of old current equipment and installation of new equipment. Line Item 0007, Training for the new equipment to VA personnel on operation of the equipment: Quantity: 1 Line Item 0008, Service Manuals: Product Number: 40225672, Two copies of all user, parts, service and maintenance manuals for each piece of equipment ordered. THIS SOLICITATION MUST BE QUOTED ALL OR NONE. SUBSTITUTE ITEMS: Due to servicing and warranty requirements only pricing for Karl Stroz equipment will be accepted. Signed and approved justification is on File. Final decision as to award will be based on lowest price. All service, training and maintenance requirements must also be met. Technical specifications of required item are attached. To be delivered to: VA Medical Center, 500 West Fort St., Bldg 34, Boise, Idaho 83702. The selected Offeror must comply with the following commercial item terms and conditions. FAR 52.212-1, Instructions to Offerors - Commercial, applies to this acquisition. The selected Offeror must submit a completed copy of the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items. FAR 52.212-4, Contract Terms and Conditions - Commercial Items, applies to this acquisition. The following FAR clauses in paragraph (b) of FAR clause 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, will apply: 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.225-13, 52.232-34.
