- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The USPFO for KY has a requirement for Physical Health Assessments (PHA) Locations and specification can be found in the attached Statement of Work (SOW). This solicitation will be awarded to the Service Disabled Vetran Owned Small Bussines with the lowest price technically acceptable. Evaluation criteria will include Past performace, technical and Price with all being equal. Closing date for RFP will be April 29th 2010 at 2pm ET.
