- HCR 13668 Forms & Instructions.zip contains the forms that must be downloaded, completed and returned when submitting an offer and the other information potential suppliers need to take into account to formulate an offer. HCR 13668, Sol 060-74-10 Statement of Work contains the details of the route potential suppliers will need to take into account to formulate an offer. Route #- 13668 Termini- Potsdam, NY to Chase Mills, NY Response Deadline- 05/07/2010 @ 15:00 ET Contract Term- 06/01/2010 through 06/30/2013 This is a three year, regular, renewable, fixed price contract to provide service in accordance with Terms & Conditions Issue 9, 2/26/10 and HCR 13668, Statement of Work 060-74-10. Prospective offerors will need to take into account all instructions and references in solicitation package HCR 13668, Solicitation 060-74-10 in order to formulate an offer. Offerors will need to open and print the attachment labeled "FORMS THAT MUST BE COMPLETED & RETURNED".
